SaintA Hiring For Service Facilitator at West Bend, WI  Full Time

Job Purpose:
The Service Facilitator is responsible for the coordination of services for youth in the Comprehensive Community services program of Washington County and to ensure needs of youth and family are met by providers to facilitate greater overall mental health and well-being.

Duties:

• Performs clinical assessments, and develops and implements client treatment plans
• Provides individual and family psychoeducation and skill development
• Develops a network of community support and resources, connects clients with resources as appropriate or makes referrals as needed
• Establishes and maintains working knowledge of WCHSD programs, resources and supports
• Facilitates team meetings
• Maintains client files and participates in quarterly review of files
• Monitors outcome data to measure program performance

Qualifications:

• Bachelor’s degree in Social Work or equivalent required
• 2 years’ experience in child and family services
• Ability to work within a multi-disciplinary team
• Excellent written and oral communication skills
• Exceptional organizational skills

Position Details:

• Full time, Hybrid work status with the opportunity to work remotely
• Flexible schedule, includes some evenings and in-home hours
• Reports directly to the Operational and Clinical Supervisor
• Must have own transportation, meet agency auto insurance requirements and have a valid driver’s license with an acceptable driving record
• Primarily located in our West Bend office
