The Department of Public Health is recruiting for Community Health Workers (CHW)* who assist and support clients in clinical and non-clinical settings. A CHW is the bridge of support for individuals in the community, to improve health understanding, access and outcomes. CHWs will establish a trusting and safe connection with clients for the purpose of assisting in navigating through provider care plans.

CONDITIONS OF EMPLOYMENT
Pre-Employment Process: Prior to appointment, applicants must pass a background investigation which includes verification of employment history and education, fingerprinting, physical exam, and drug screening.
Travel: Travel throughout the County may be required. At the time of hire, a valid California driver license and proof of automobile liability insurance must be produced for the individual providing the transportation.
Basic Life Support Certification: Positions within Clinical Operations may require a Healthcare Provider level Basic Life Support (BLS) certification.
Sponsorship: Please note San Bernardino County is not able to consider candidates who will require visa sponsorship at the time of application or in the future.
Community Health Worker Trainee: Trainees must be promoted to the journey-level classification, Community Health Worker (31A) upon obtaining a Community Health Worker certification, successful completion of a trainee period and a successful work progress report or be terminated.

English/Spanish bilingual skills are highly desired. For consideration of bilingual assessments, applicants must indicate bilingual ability on the application. Employees may receive additional compensation for utilizing such skills when allocated to a bilingual position, as approved by the department. Bilingual testing may be required.
