San Bernardino County Hiring for Community Health Worker* at San Bernardino County, CA Full Time

The Department of Public Health is recruiting for Community Health Workers (CHW)* who assist and support clients in clinical and non-clinical settings. A CHW is the bridge of support for individuals in the community, to improve health understanding, access and outcomes. CHWs will establish a trusting and safe connection with clients for the purpose of assisting in navigating through provider care plans.

• Community Health Worker Trainee – 5M – Hourly Rate Range $17.15 to $22.72
• Community Health Worker – 31A – Hourly Rate Range $18.01 to $24.75

Duties may include:

• Facilitating referrals and appropriate paperwork for clients to specialty clinics and treatment centers.
• Consulting with physicians and other members of the care delivery team on the client’s treatment plan.
• Promoting and providing education and awareness related to the treatments and services provided to clients, families, staff and the community in a culturally appropriate manner.
• Promotion and resource support for substance use disorder, violence awareness and prevention, preventive healthcare, and chronic disease management.
• Tracking client outcomes and relevant data using manual and automated systems.
• Guiding clients in the application process for financial healthcare coverage.
• Building and maintaining a positive and professional working relationship with healthcare teams, public health staff, and external partners.
• Ensuring documentation is accurate, pertinent and in compliance with department protocols and policies.
• Assisting clients with scheduling appointments and navigating social service systems as indicated by the shared care plans.
• Recognizing and reporting signs and symptoms of abuse and neglect to providers. Completing all mandatory reporting requirements, as needed.
• Supporting clients in their homes, community, or clinic setting and conducting outreach and engagement.
• Maintaining confidentiality of protected health information per the Health Insurance Portability and Accountability Act.

CONDITIONS OF EMPLOYMENT

Pre-Employment Process: Prior to appointment, applicants must pass a background investigation which includes verification of employment history and education, fingerprinting, physical exam, and drug screening.

Travel: Travel throughout the County may be required. At the time of hire, a valid California driver license and proof of automobile liability insurance must be produced for the individual providing the transportation.

Basic Life Support Certification: Positions within Clinical Operations may require a Healthcare Provider level Basic Life Support (BLS) certification.

Community Health Worker Trainee: Trainees must be promoted to the journey-level classification, Community Health Worker (31A) upon obtaining a Community Health Worker certification, successful completion of a trainee period and a successful work progress report or be terminated.

Minimum Requirements

For the Community Health Worker (31A) position, applicants must possess:

• Certification: A Community Health Workers (CHW) Certification. A legible copy of the CHW certificate must be attached to the online application or summitted by the filing deadline. – AND –
• Experience: One (1) year of experience providing healthcare, mental health, social or human services. Experience must include one of the following: performing needs assessments, conducting interviews, referring clients to services or performing crisis interventions.

For the Community Health Worker TRAINEE (5M) position, applicants must possess:

• Certification: Successful completion of a Community Health Workers (CHW) Certification program resulting in a certificate within one (1) year of employment. – AND –
• Experience: One (1) year of paid or volunteer experience in a mental health, substance use, social, or human services program.

Desired Qualifications

The ideal candidate will possess the following knowledge/skills/abilities:

• Ability to communicate effectively in person, in writing, and on the telephone.
• Be organized with attention to detail.
• Possess critical thinking skills.
• Be culturally sensitive to the population being served.
• Any combination of lived experience working with diverse populations in the community (e.g., women with infants and young children; persons experiencing housing instability, food insecurity, unemployment, interpersonal violence, neglect, racism, discrimination, HIV/AIDS, substance abuse; persons involved in the justice system; older adults, underserved populations; or other circumstances that could prevent a person from achieving their optimal wellness).
• Possess basic understanding of medical terminology.
• Possess general knowledge of Microsoft Office Suite (Outlook, Word, Excel, and PowerPoint).

English/Spanish bilingual skills are highly desired. For consideration of bilingual assessments, applicants must indicate bilingual ability on the application. Employees may receive additional compensation for utilizing such skills when allocated to a bilingual position, as approved by the department. Bilingual testing may be required.
