- Serves as a medication expert to support SSCHC providers and provide care to SSCHC patients.
- Identifies and implements clinical pharmacy services that will be a value add to clinic staff and patients.
- Engages with SSCHC clinical leadership and participates in decision-making processes related to the implementation of new clinical processes related to medication therapy.
- Engages with SSCHC clinical leadership to ensure appropriate use of clinic administered medications that meets Joint Commission standards.
- Oversee SSCHC Clinical Pharmacy Residents in conjunction with Schools of Pharmacy.
- Oversee SSCHC Clinical Pharmacy staff.
- Assists in updating SSCHC 340B Formulary, educate clinic staff of formulary medications, and assists in review of formulary exemptions.
- Conducts Annual Wellness Visits for eligible patients.
- Provides direct patient care to patients referred to clinical pharmacy program.
Specific Responsibilities:
- Provide direct oversight of growing SSCHC clinical pharmacy team.
- Work with clinic leadership to ensure successful integration of clinical pharmacists within our care teams.
- Provide vision for expanded roles of clinical pharmacy within SSCHC.
- Recruitment of highest quality clinical pharmacist candidates.
- Development training programs and clinical pharmacist development.
- Oversee relationships with Schools of Pharmacy to recruit and oversee clinical pharmacy fellows, residents, and students to ensure strong recruitment pipeline.
- Oversee expansion and updates of collaborative practice agreements.
- Oversight of medication use within the clinic in compliance with Joint Commission standards.
- Development of medication safety event program.
- Assessment of population health data within clinic to provide targeted interventions to optimize therapy (statin use, albuterol use as examples).
- Provide clinical pharmacy services to patients, emphasizing health promotion and disease prevention as well as management of acute and chronic health conditions.
- Provide medication education, health promotion, and disease prevention to patients.
- Recommend or adjust medication prescriptions in accordance with a collaborative practice agreement.
- Provides drug information and patient care consults to other health care professionals upon request.
- Participates in SSCHC’s Quality Improvement initiatives.
- Assists in the development of educational materials for patients and SSCHC providers.
- Participates in the creation of ambulatory policies and procedures as requested.
Qualifications:
- Licensed as a pharmacist in the State of Wisconsin, with license in good standing.
- Insurable for liability coverage as a pharmacist.
- Proven high level written and oral communication skills.
- Self-motivated and independent, able to work without direct supervision.
- Ability to process large amounts of data.
- Ability to work well within a team.
- Minimum 5 years’ experience as a clinical pharmacist or in clinical pharmacy leadership.
- Previous experience acting as site coordinator for a clinical pharmacy residency program.
- Ability to relate well to people from diverse ethnic and cultural backgrounds.
