Sixteenth Street Community Health Centers Hiring For Registered Nurse (Augustine Prep In-School Clinic) at Milwaukee, WI  Full Time

Job Responsibilities:

1. The school nurse works in collaboration with the school-based clinical team members to coordinate and provide health services to students and serves as the health resource to staff members to enhance health and wellness in the school community.
2. Serves as a leader in health promotion and disease prevention and in coordination with the school leadership, works to maintain a safe school environment.
3. Provides patient-centered nursing care in an outpatient school-based clinic, according to established scope of practice guidelines and standards of care.
4. Provides triage assessment to walk-in patients, their family members, school staff, and others for referral to Provider. Communicates with Providers and other members of the health care team.
5. Educates students, staff, and others on health topics to promote health, prevent disease, and minimize the impact of disability. This might occur in the classroom setting or on an individual basis.
6. Leads in the development of policies, programs, and procedures for the provision of school health services at an individual or school level, relying on student-centered, evidence-based best practices.
7. The school nurse uses clinical judgment to assess and treat students’ illnesses and injuries. Administers medications, under Provider orders, including but not limited to: Immunizations, antibiotics, pain medications, topical agents, insulin, and allergy antigens. May occasionally initiate, maintain and discontinue IV therapy.
8. Assesses and provides basic to complex patient education and counseling on the performance of tasks, skills, treatment, and self-care, integrating health promotion and preventive aspects of care. Follows through with necessary procedure or test requests, prescription refills, new appointment times, and referrals to other facilities or services. Refers questions to Provider and relays information back to the patient as directed.
9. Performs nursing tasks or skills such as removal of sutures and staples, basic wound care, catheterization, audiograms, laboratory procedures (throat/nose culture, drug screen, phlebotomy – butterfly, venous access), spirometry, basic wound care, etc., with demonstrated competency.
10. Prepares and rooms the patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e., chief complaint, allergies, medications). Assists the physician/provider with procedures.
11. Executes nursing interventions through collaborative work with school-based providers and support staff. Implements health protocols and student tracking systems as needed.
12. Organizes school health office and manages student’s health plan and medication logs.
13. Collaborates with school staff reviewing students’ immunization records to ensure State compliance.
14. Assists with conducting vision and hearing screenings.
15. Acts as an expert clinical and department resource to Medical Assistants. May provide work direction to other staff and/or assist with patient care flow and the provision of services. Proactively participating in staff development by seeking to provide “on-the-spot” learning opportunities, creating in-servicing plans, and delivering in-servicing plans for support staff.
16. Assists with telephone triage and other RN pool responsibilities, as needed.
17. Assist in maintenance/protection of patient privacy and medical record data collection by:

• Proactively upholding clinic guidelines and standards related to Patient Confidentiality.  
• Review patient data on the chart each visit for necessary health maintenance issues and/or follow-up issues from previous visits.
• Rendering complete, accurate, and professional documentation of care rendered, procedures performed, tests administered, tasks completed, results received, and/or other patient-related information pertinent to care.

18. Participate in the Clinic’s mission to provide high-quality healthcare by:

• Actively participating in department In-servicing and Annual Skills Testing.
• Actively seek independent studies to keep aware and current on nursing issues. 
• Alerting appropriate staff and/or acting on possible safety or confidentiality issues.
• Offering workflow, safety, morale related improvement suggestions.

19. Maintain additional assigned clinical projects (staff development, Coumadin clinic, etc.) in a safe, orderly, and documented manner, with minimal supervision requirements.
