Job Description
Overview:
St. Luke’s Health System in Meridian is seeking a RN Care Coordinator / Health Coach to join our Lifestyle Medicine Clinic team.
The RN Care Coordinator / Health Coach plays an integral role in the operations of the Lifestyle Medicine program. The RN Care Coordinator/Health Coach will follow up on patient medical concerns, work as part of a multi-disciplinary team, assist the patient to develop wellness vision and help patient navigate across difference mediums (virtual video, tele, in-person, and be well versed in both individual and group health coaching techniques.
Hours: Primarily day shift, Monday – Thursday. Occasionally includes evening support.
Responsibilities:
- Works as clinician, consultant, educator, and researcher to facilitate collaborative management of patient care.
- Works closely with physicians, nurses, social workers and a wide range of medical and non-medical professionals.
- Ensures patient is progressing towards desired outcomes by monitoring care through assessment and/or patient records.
- Determines need for therapeutic medical, psycho-social, and psychiatric evaluations. Reviews therapist evaluations, treatment, objectives, and plans.
- Develops patient care plans, evaluates and monitors progress, responds to patient and family needs. Serves as advocate when problems and issues arise.
- Coordinates discharge and post-discharge requirements; orientates and trains family members; provides resources.
- Performs therapeutic nursing interventions under the direction of a licensed physician.
- Provides ongoing patient education on diagnosis, treatments, prognosis and medications.
- Perform other duties and responsibilities as assigned.
Qualifications:
- Education: Bachelors’ of Nursing Degree
- Experience: 2 years’s of RN experience
- Licenses/Certifications: Current RN License in the State of Practice AND American Heart Association (AHA) BLS Provider certification
Preferred Qualifications:
- Familiar with the practice of Lifestyle Medicine and its’ 6 pillars.
- Previous care coordination and/or case management experience with chronic disease patients.
- Board Certified Health Coach (HWC-NBHWC or HWC-ICF)
- Experience with group health coaching
