This position serves as the expert for the Care Transition Program providing guidance on aging network accessible housing and home repairs focusing on state and national housing programs, policies, and procedures to support the statewide hospital to home care transition initiative. Provide monitoring and oversight to regional AAAs that administer social/health related and nutrition services to older adults in Michigan. This position functions as the designated aging network accessible housing and home repair specialist focusing on state and national housing programs, policies and procedures to support the statewide hospital to home care transition initiative. Responsibilities also include providing oversight of assigned Area Agencies on Aging (AAA) and aging network grantee agencies. Position provides policy interpretation to local area agency policy boards and executive directors. The position identifies issues that require State Commission approval. The position provides oversite AAAs in resolving budget and program issues.

Required Education and Experience
Education
Possession of a bachelor’s degree in any major.
Experience
Four years of professional experience, including two years equivalent to the experienced (P11) level or one year equivalent to the advanced (12) level.

Position Location/Remote Office: The office location for this position is at 333 S. Grand however, this position has the ability to work hybrid (remote/office). Selected candidates who have been approved to work remotely or work hybrid must complete that work within Michigan. Candidates should confirm work location and schedule at the time of interview.
Employees will be provided computers to perform state work. Phones may also be provided for necessary communications. If working remotely, employees will be responsible for providing other components of a remote office at their own expense, including:
A secure work location that allows privacy and prevents distractions.
A high-speed internet connection of at least 25 Mbps download and 5 Mbps upload.
Suitable lighting, furniture, and utilities.
