Cordilleras is a licensed 68-bed mental health rehabilitation center (MHRC) and a licensed 49-bed adult residential facility (ARF) called The Suites. Cordilleras primarily serves San Mateo County residents, 18-59, with long histories of mental illness and multiple episodes of acute psychiatric hospitalization.

Shifts Available:

Full Time; AM 8:00 am – 4:30 pm; Monday – Friday

Minimum Compensation: $28.45 Hourly

POSITION OBJECTIVE

Under supervision of a licensed clinician, the Social Work Clinician I provides general casework services to members served and natural supports. This involves person-centered recovery planning and collaborating with other services and agencies.

QUALIFICATIONS

A Bachelor’s Degree in Social Work or psychology from an accredited college or university and a valid California Driver’s License are also required. Necessary skills include the ability to work effectively as a member of a multidisciplinary team, a thorough understanding of client rights, an understanding of the conservatorship process and knowledge of basic medical and psychiatric terminology. Criminal justice clearance and education verification are required.

• Assumes overall responsibility for discharge planning, including:

1. Assessing the clients readiness for discharge in coordination with the Treatment Team.
2. Making appropriate referrals for placement, case management and aftercare services, as assigned.
3. Coordinating with the Business Office to assure that resident benefits and finance are in order, as assigned.
4. Developing and maintaining an awareness of community resources.

• Assures that clients know and understand their legal status and rights. Tracks legal status on all assigned clients.
• Provides consultation to clients and their families regarding issues arising from the clients’ admission, discharge and/or aftercare plan.
• Provide direct treatment for assigned clients and families.

TEAM MEMBER PARTICIPATION

• Participates as a team member and provides input via reporting observations, concerns and asking appropriate questions.
• Reviews and implements resident treatment plans.
• Actively facilitates and participates in multidisciplinary team meetings and treatment planning meetings.
• Demonstrates knowledge of Professional Assault Response Training (PART) principles and uses PART concepts to prevent and safely manage aggressive behavior.
• Provides leadership in facilitating the team process.
• Provides education/training to other team members as necessary.

ASSESSMENT

• Observes, records and reports resident social and psychiatric behavior.
• Completes the Psychosocial Assessment for assigned clients. This includes obtaining necessary data regarding the clients social and psychological history.

• Demonstrates knowledge and proper use of equipment and supplies.
• Promotes and assists clients in developing self-help skills.
