Job Description

General Responsibilities:
The HEZ Prevention Community Health Worker (CHW) is a member of the Health Equity Zone of Woonsocket team. The CHW works with the Family Planning and Sexuality Educator Supervisor to promote healthy teen development through outreach and by providing teens with the tools, knowledge, and/or skills they need to make healthy and responsible decisions concerning their sexual and reproductive health. The Community Health Worker plays a critical role within the community by supporting care coordination, outreach, community resources and referrals. This is a grant funded position thru August 31, 2024.

Major Responsibilities:

• Work directly with teens to implement best practices, assess and research education/training needs, develop materials and training activities related to teen reproductive health and human sexuality.
• Provide education and training in both large and small group settings.
• Provides evidence based educational/informative presentations, curriculums, and training to teen groups as well as other community groups and professionals on issues related to teen reproductive health, human sexuality, and promoting healthy sexuality, and as required by Title X.
• Health education, outreach and referrals for individual teens to community resources and school based health center/community health center.
• Conducts professional development trainings for teachers, school support staff, nurses, parents, volunteers, teen leaders, and other facilitators in teen reproductive health and human sexuality.
• Adjust communication and presentations based on situation/audience, as appropriate.
• Track and report data for program evaluation, maintains up-to-date records and data in HEZ specific database and in patient record as appropriate.
• Tracks and completes records, reports and documentation of all training in accordance with Title X programs and other grants and funding sources.
• In conjunction with supervisor, researches, collects, analyzes and reviews training tends, statistical data, and reports for improving school/community education resources; monitors training methods to ensure resources are current, effective, and available to staff and community members in a timely and cost-effective manner.
• Attend relevant community meetings in HEZ and Rhode to Equity.
• Must be adaptable to changing situations, schedules and local travel.

Physical Effort/Environment:

• Meet with Woonsocket youth in school or at other community settings.
• Moderate in-city travel to present or meet with community groups and/or partners. As needed, travel to neighboring cities for meetings.
• General office environment: manual dexterity needed for keyboard work, writing and phone calls.
• Use of technology for virtual learning.

Communication Skills:

• Strong communication skills, both oral and written, required.
• Ability to relate and build trusting relationships.
• Ability to communicate effectively with co-workers, youth, family/care givers, and partner agencies.
• Bilingual/multilingual candidates strongly preferred (Spanish preferred, all languages welcome).

Confidentiality of Information:
A high level of confidentiality. Limited access to medical records. Must always maintain all policies of confidentiality and use discretion surrounding agency information and management issues.

Work Schedule Demands:
Full-time. Ability to work evenings and weekends as required.

Required Qualifications:

• Community Health Worker (CHW) certification or willingness to become certified within 6 months or as soon as possible based on course availability. (All training and certification costs will be paid by RI Department of Health.)
• High School diploma/equivalent and a combination of education and skills to effectively carry out responsibilities and assignments, such as previous experience working in a community-based setting.
• Ability to work independently and effectively in a team environment
• Demonstrates ability to provide education and training in both large and small group settings.
• Highly organized and strong interpersonal skills.
• Maintain timely and accurate records and documentation as required.
• Demonstrated experience working with computers and other technology and Microsoft Office applications.

Recommended Qualifications:

• Experience providing LGBTQ inclusive sexuality education and reproductive health education.
• Experience working with diverse populations and local community groups.
• Relevant teaching experience in a classroom or community education setting.
• Resident of Woonsocket and/or commitment to the community.
