General Responsibilities:
The Integrated Behavioral Health Clinician as part of a multidisciplinary treatment team, identifies, triages, and manages patients with medical and behavioral health problems within the primary care setting. The IBH Clinician will assist primary care providers with conducting screens, diagnosis, assessment, and brief treatment or referral of patients with behavioral health concerns. The IBH Clinician will facilitate the integration of primary and behavioral health care through care coordination and case consultation.

Duties and Responsibilities:

• Consults with primary care providers in real time to assist patients with psychosocial issues. This is frequently done as a “warm hand off” during hours of clinical operation. Informal consultations and referrals from the primary care providers happen at other times as well.
• Assist the primary care provider in recognizing, treating and managing mental health and psychosocial issues and acts as a contributing member to the primary care team
• Provide brief treatment interventions for mild to moderate behavioral health and crisis intervention. Demonstrate competence in brief assessment, crisis management, triage, diagnosis, as well as, mental health/substance abuse treatment within a brief solution-focused model.
• Develop and maintain collaborative working relationships with community providers. Initiate linkages, disseminate information and make referrals to the appropriate services as needed.
• Conduct brief screenings to evaluate the needs of adults, adolescents, and children for behavioral health intervention, case management, or ongoing mental health or addiction treatment
• Provide clinical and consultative treatment services that are strength based and client centered.
• Coordinate with clinical providers, social service providers, and care coordinators
• Document all aspects of the screening, diagnosis and treatment or referral in the EHR
• Complete all required documentation for treatment planning, billing, and licensure within a timely manner
• Participate in professional activities such as: continuing education activities, clinical conferences, meeting with representatives of community groups
• Ensure patient confidentiality at all times
• Perform other necessary duties as required by Thundermist Health Center to achieve the goal of providing high quality behavioral health services
• Participate in on-call BH coverage

Qualifications and Skills:
Knowledge

Demonstrates a thorough understanding of DSM-V as measured by thoroughness and accuracy of diagnostic formulations.

• Knowledge and ability to independently carry out behavioral health screening, diagnosis, assessment, treatment planning, and treatment or referral of behavioral health patients
• Demonstrate a thorough knowledge of effective treatment protocols for brief solution-focused interventions and necessary cultural competencies for treating children and adults with mental health/substance abuse issues, as well as chronic disease lifestyle management.
• Adapts therapeutic strategies to individual characteristics of the patient, including but not limited to, disability, gender, sexual orientation, developmental level, culture, ethnicity, age, health status as measured by return visits and patient satisfaction.
• Previous experience in a primary care setting, ideally in a federally qualified health center (FQHC)
• Ability to work in a high energy, quickly paced environment and maintain a professional demeanor and attitude
• Excellent interpersonal and communication skills with patients, their families, internal and external staff
• Knowledge of HIPPA regulations
• CPR certified

Technical Qualifications

• Master’s degree (M.A., M.F.T., M.S.W.) MSW Strongly Preferred
• Experience treating patients with comorbidities
• Must be eligible for RI State Licensure: LICSW, LMFT, or LMHC. LICSW Strongly Preferred
• Oral and written proficiency in English. Bilingual Spanish Strongly Preferred
• Computer skills including word processing and data entry required. Previous experience with eClinicalworks Strongly Preferred

Communication Skills:
1. Strong communication skills, both oral and written, required.

2. The ability to effectively work and communicate with a multi-disciplinary team, other services providers, and patients is critical to the success of this position.

3. The development and submission of written reports accurately detailing service provision is required.

Confidentiality of Information:
The clinician in this role has access to client’s medical records, financial information, and documentation regarding prior services from outside agencies. The maintenance of client information confidentially is required. The incumbent may have access to information regarding evaluation of the resident’s performance and confidentiality is required.
