General Responsibilities:
The Health Equity Zone Project Manager facilitates hyper-local, community driven, collective impact initiatives that address barriers to health with the community. The project manager understands the concepts of social determinants of health and institutional inequities, including racism, classism, homophobia, transphobia, weight stigma, stigma surrounding substance use, and other social constructs that impact health. The project manager ensures the interventions and strategies align with the community’s priorities, are social justice oriented, and improves opportunity for health for those who are marginalized. The HEZ manager develops high functioning partnerships with teammates, residents, and organizations to: a) identify and address community needs and priorities, b) build and manage measurable and attainable multi-year work plans to improve health outcomes and equity c) develop and manage shared goals to foster collective impact d) manage budgets and subcontracts e) ensure grant deliverables and reporting requirements are met on time. Position requires exceptional communication, organization, and project management skills. The project manager supervises the HEZ team in their assigned community.

Must maintain confidentiality at all levels of interaction, some access to clinical and fiscal information. Must use discretion surrounding agency information, inter-agency communications/negotiations, and management issues.
