Location: 100% REMOTE (Must reside in MI)
Schedule: Mon-Fri (job dependent hours)
Job Description/Requirements:
Provide support to medical, behavioral and social support staff to determine needs, eligibility and services for members. Conduct screening and risk assessment interventions per program guidelines.
Experience/Education:
- Requires an active and clear RN, LPC, or LMSW, License in MI
- Requires previous experience as a Care Coordinator, Case Manager, Medical Social Worker, or Registered Nurse
