Touchstone Health of San Antonio is actively seeking a qualified and compassionate Hospice Registered Nurse (RN) Case Manager to care for our patients. The ideal candidate has excellent clinical skills, a passion for direct patient care, and the ability to function well autonomously. Duties & Responsibilities:
- Evaluate and treat patients using the most current technology and practices
- Develop initial plan of care based on communication with the physician and patient assessment
- Participate in clinical outcomes monitoring, follow up, and agency performance improvement initiatives
- Manage and educate Hospice Aides and LPNs performance in implementing nursing services
- Collaborate with an interdisciplinary team to provide highest level of care
Requirements
Qualifications & Requirements:
- Current and unencumbered license to practice as a Registered Nurse in the state of Texas
- Minimum of one (1) year of hospice nursing experience
- Current CPR certification
- Reliable transportation, valid driver’s license with no DUI’s, revocations, or suspensions in the last 5 years
- Exceptional customer care
- Excellent written and verbal communication skills
- MS Office skills (Word, Excel, PowerPoint)
