The Integrated Health Clinic Nurse will perform duties of a Registered Nurse in Trilogy’s integrated healthcare clinic. The Clinic Nurse provides assessment and triage, care coordination, and health education to patients in a culturally appropriate and relevant manner, using motivational interviewing and other evidence-based strategies. The Clinic Nurse will collaborate with the interdisciplinary treatment team on client plans of care to ensure quality standards are met. The Integrated Health Nurse will provide strengths-based, client-centered, and trauma-informed services to clients with serious mental illness, substance use disorders, and medical comorbidities utilizing a harm reduction approach.
Responsibilities:*
Work with designated Federally Qualified Health Center partner organization’s team to provide clinic nursing services and serve as a liaison with Trilogy staff and teams

• Assess clients’ presenting health problems and prioritize for medical attention using accepted triage principles
• Provide individualized direct nursing care to clients; this includes but is not limited to health education, coordination of care, promotion of psychiatric and medical illness self-management skills, and administering injections of long-acting antipsychotic medication
• Advocate for clients, communicating effectively with their Trilogy team members, and linking them to needed community services
• Provide triage services to clients to assess need for urgent care with providers or referral to higher levels of care
• Collaborate with Trilogy and partner organizations’ psychiatric providers to support treatment plans. This may include administering psychiatric rating scales, assessment of medication adherence, and coordination with external providers and family members
• Collaborate with partner organization’s primary care providers and other clinic employees on patient plans of care to ensure quality standards are met
• Support a population health management approach to nursing care using client data on chronic health conditions and health behaviors and assessment of health literacy
• 4 days per week in the clinic and 1 day per week at a residential site
• Support nursing students and students from other disciplines as assigned
• Perform other related duties and/or projects as assigned.

Qualifications:

• Must have received or be willing to receive the COVID-19 vaccination and booster by the date of hire to be considered. Proof of vaccination required.
• Associate’s Degree in Nursing required, Bachelor’s of Science in Nursing or MS/MSN/MPH preferred
• Licensed Registered Nurse (RN) in the State of Illinois
• Experience working with people with serious mental illness and medical comorbidities preferred
• Experience in community-based services highly preferred
• Exceptional communication and customer service skills and ability to interact with a diverse group of partners
• Valid Illinois driver’s license with daily access to a well-maintained vehicle with $1,000,000/$3,000,000 liability insurance
