President, Saint Alphonsus Health Alliance, Clinically Integrated Network
POSITION PURPOSE
Under the general direction of the Saint Alphonsus Health System (SAHS) CEO and A-2 relationship with the Trinity Health SVP of Clinical Integration and Physician Services, provides leadership and direction for the strategic vision of population health and value-based care at SAHS. Serves as the Saint Alphonsus Health Alliance (SAHA) Clinical Integrated Network (CIN) president and the leader of value-based care to achieve these strategic priorities.
ESSENTIAL FUNCTIONS
Purpose and Leadership
1.Meets Saint Alphonsus Health System Guiding Behaviors and Caring Standards including interpersonal communication and professional conduct expectations.
2.Serves as the SAHS and Trinity Health executive leader responsible for creating and implementing the SAHS population health strategic plan across the continuum. Participates in relevant SAHS executive and Trinity Health meetings that facilitate the implementation of this strategy.
3.Leads through influence and collaboration across continuum stakeholders to achieve results.
4.Provides executive leadership of SAHA’s broad network of providers that includes independent affiliates and community providers and the Saint Alphonsus Medical Group (SAMG) providers. Responsible for the quality, cost, and the member experience for patients covered by health benefit plans that contract with SAHA.
5.Ability to provide leadership and direction for SAHA while recognizing and supporting the overall SAHS strategy and culture.
Business Operations
6.Responsible for development and effective performance of the SAHA management team.
7.Responsible for effective operational support and achievement of SAHA governance committees Effectively utilizes SAHA governing board, committees, and task forces to achieve strategic vision and goals.
8.Responsible for development of SAHA in accordance with the strategic and business plans.
9.Responsible for creation of value to the physician participants in the SAHA Network that includes physicians, advanced practice providers, SNFs, home health, wrap around service providers.
10.Responsible for implementation of the Saint Alphonsus Medical Group’s strategy for success in all value-based agreements with SAHS’s contracted payers.
11.Accountable for financial performance of SAHA consistent with the annual budget and long-term business plan as approved by SAHA and SAHS System Board.
Clinical Model
12.Works with the SAMG executive team to implement new models of care delivery that utilize team-based care, and remote patient management to reduce potentially avoidable healthcare utilization and improve performance on quality measures. Understands the use of technology to support care including remote patient monitoring, member outreach, telehealth services, and ancillary technology partners.
13.Works with the SAMG executive team and Trinity Health to optimize the functionality of EPIC TogetherCare for population health management. Population Health IT platform(s) to supplement TogetherCare
14.Ensures effective collaborating across the Alliance, SAMG and Community Health and Well Being to address social determinants of health.
15.Responsible for identification and implementation of quality of care, safety and efficiency initiatives that result in measurable improvement in care, safety, and cost of care in the community.
Alternative Payment Models
16.In close association with the SAHS executive team and system Payer Strategy and Product Development team, seek to develop innovative payment models that recognize value as basis for payment.
Analytics
17.Understands the importance of clinical and health claims data to drive improvement activities. Works with SAHS business intelligence and Trinity Health data analytics teams to implement platforms that provide tactical and strategic information for effective management.

MINIMUM QUALIFICATIONS
1. Masters in Health Care Administration, Master in Business Administration or similar another advanced degree and previous experience in population health and physician group collaboration as a Clinical Integrated Network (CIN) or Accountable Care Organization (ACO) executive.
2. Respected executive who has the executive presence and exceptional skills in leading and working with health care providers and systems in the areas of quality, performance improvement and organizational development.
3. Previous Senior leadership experience in a Clinical Integrated Network (CIN), ACO, or medical group practice from an operational, financial, and regulatory standpoint.
4. Previous experience in managing as staff lead for a governance Board. Ability to partner with a board chair, set strategy, monitor performance, remove barriers, and set policy
4. Prior responsibility and accountability for achievement of organizational results in Population Health space including, but not limited to, Risk Management, Contract Management, Risk Scoring, Quality, Safety, and financial performance. Medical group and/or health plan?
5. Proven results in working to drive outcomes in a collaborative and effective manner with independent Affiliates, Community Providers, and Trinity Health Medical Group/SAMG.
8. Ability to lead large scale organizational change including major initiatives in the payment, technology, and physician work‐ force management areas. Demonstrated competence in work force management.
PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS
1. Must be able to adapt to frequently changing work priorities and be able to prioritize and balance the requirements of working with the System Office and Health Ministries (RHMs).
2. Must be able to travel to the various SAHS sites and Trinity Health sites as needed.
