POSITION SUMMARY
As the Care Coordinator for the Traditional Wellness Program, the individual will be involved the development of a Traditional Wellness Program. They will be responsible for assisting clients as a traditional advisor. They will provide one on one traditional wellness support for individuals. They will provide talking circles, traditional workshops and teachings. The Traditional Care Coordinator will be a part of a multi-disciplinary team and most services are provided in the field.
ESSENTIAL DUTIES, FUNCTIONS & RESPONSIBILITIES
Office/Field Work
* Possess a reliable vehicle with valid CA driver’s license and active auto insurance
* Office or In-home field based traditional support for clients with a range of psychological, emotional and behavioral problems
Client Counsel/Advisor
* Communicate with newly assigned clients within 24 hours and initial visit within 5 days
* Provide appointments with clients for one on one traditional wellness counseling
* Provide talking circles
Community Support
* Outreach to the community to promote awareness of the programs available at UAII on a regular basis
Activities Management
* Provide direct services such as sage picking classes, traditional medicine classes, traditional crafts, traditional teachings and other needed traditional services
* Coordinate and implement cultural activities for Individuals in the Traditional Wellness program
* Manage subcontractor’s activities under the Prevention and Aftercare program
Administration and Meetings
* Submit work schedules and productivity reports on weekly basis to assigned supervisor
* Able to submit all completed paperwork to supervisor within 24 hours after seeing client
* Prepare reports regarding clients’ care and progress as required
* Update client charts, as new information is obtained
* Review annually (base on admission date) client care coordination plans and client chart forms
* Communicate with colleagues and treatments teams about any and all client information within 5 business day
* Attend all mandatory weekly staff meetings, staff trainings, treatment team meetings, and supervision meetings
* Provide feedback to treatment teams on traditional care coordination issues
* Address and employ supervisors’ assistance with any and all client crises
* Communicate with supervisors regarding absences from work and plan for client and work task coverage
* Keep Director well-informed regarding activities, pending issues, and potential problems
MINIMUM QUALIFICATIONS
Experience & Qualifications:
* Demonstrated experience of a minimum of 2 years in the provision of traditional healing, traditional advising care coordination in a human service setting.
* Knowledge of American Indian traditional teachings.
* Known in the community as a traditional advisor.
Required Education:
* High School Diploma or Associate of Arts degree from an accredited institution

* N/A
Confidentiality:
* Maintain strict confidentiality of client and personnel information adhering to HIPPA, professional codes of conduct, State of CA regulations, Privacy Act, and UAII policies and procedures
* This position has access to sensitive information, and a breach in confidentiality will be grounds for disciplinary action up to and including immediate termination
Communication/ Interpersonal Skills:
* Possess the ability to communicate with others in a courteous and professional manner
* Excellent customer service skills
Technology Skills:
* Knowledgeable in Google
* Knowledgeable in Microsoft Word
Other Skills:
* Available and willing to travel as needed, travel time could be from 30-40%
* Available to work evening and weekend hours as needed
* Willingness to learn new skills and participate in training sessions relevant to the position
POSITION PREFERENCES
* Experience working with the American Indian/Alaskan Native community
* Experience working with nonprofit organizations, community groups, and performance-based government contracts.
* Knowledge of Indian Self-Determination and Education Assistance Act (Public Law 93-638)

Work environment:

Physical demands:
The physical demands described here represent those that an employee must meet to perform the primary functions of this job successfully. While performing the duties of this job, the employee is required to stand, walk frequently, sit, bend, twist, talk, and hear. There are prolonged periods of sitting, keyboarding, reading, as well. The employee must occasionally lift and move up to 25 pounds. Specific vision abilities required by this job include reading, distance, computer, and color vision. Talking and hearing are essential to communicate with staff and vendors.

Mental demands:
There are several deadlines associated with this position. The employee must also multi-task and interact with various people on various and, at times, complex issues.
