POSITION SUMMARY:
To perform a variety of basic to complex record keeping tasks associated with maintenance of an automated organization medical records system. To support clinical staff in the clerical preparation of medical documents set to the physician. Oversee timely processing to ensure payment for services. To assist with more complex system planning, coordinating and troubleshooting functions.
MINIMUM QUALIFICATION STANDARDS:
I. Education:
Associate’s degree in business or other appropriate discipline
II. Experience:
One to two years of relevant clerical and technical experience, or a combination of education and experience from which comparable knowledge and skills are acquired. A minimum of 6 months to 1 year as a Technical Support Specialist I at the UVM Health network Home Health & Hospice
III. License Requirements:
N/A
IV. General:
* Must demonstrate and maintain any required competency
* Able to speak, read and write in English
* Be honest, self-directed, possess initiative, display good judgment and a positive attitude toward daily activities
* Have the ability to communicate clearly and effectively both orally and in written work
* Be able to work cooperatively and in a productive manner with organization’s employees, clients, customers and the general public
* Maintain satisfactory standing with the Adult Abuse Registry, Child Abuse & Neglect Registry, Vermont Criminal Information Center and OIG
* Be free from the illegal use and effects of drugs and alcohol in the workplace
ESSENTIAL PHYSICAL REQUIREMENTS:
Kneeling, crouching, stooping, twisting, reaching below waist level, reaching overhead and reaching torso level
Manual finger dexterity-will need dexterity to perform duties and use equipment
Visual acuity-will need sufficient eyesight to drive a car, use office equipment and perform duties
Auditory acuity- will need to assist individuals in person and by telephone
PERFORMANCE REQUIREMENTS:
I. Performance Standards:
* Enter information from clinical forms into computerized medical records system in order to prepare formal documentation used for organization compliance standards and billing purposes, including admissions, supplemental orders and re-certifications:
o Transfer chart information in proper format for billing purposes, relating care plans to primary diagnosis, goals and discharge plans, and doing all necessary coding
o Clarify questionable clinical record information with nursing staff, billing office, supervisor, or other appropriate staff member in order to ensure accuracy of medical records
o Generate medical records forms (485s)
* Process forms for mailing (supplemental orders and 485s)
* Compile recertification lists and other special logs/lists related to assigned functions
* Assist in/carry out departmental operating systems planning functions, such as improving workflow within the department and between medical records, clinical programs and the billing office
* Assist in preparation of special reports and other special projects
* Confer regularly with billing office and nursing staff to coordinate activities, clarify information, investigate and resolve problems, educate and the like
* Analyze written medical orders and enter information into the computer in all the necessary fields to ensure complete and accurate documentation
* Act as a resource to staff for questions related to OASIS, diagnosis and other processes
* Internal daily and external weekly tracking of clinical orders and production of weekly outstanding orders report
* Other duties as assigned by supervisor

* Meets own learning needs. Participates in educational programs of the organization, including completion of all mandatory competencies, education or training
* Effectively communicates to transfer information, provide guidance and resolve conflicts
* Reports concerns and complaints regarding abuse, neglect and exploitation in accordance with Vermont State Law and the organization
* Enhance or improve organization’s services by participating in team meetings, projects and committees
* Contributes to a positive work environment and responds in a constructive manner to change
* Documents the provision care and services in accordance to organization’s policy
