Position Summary:
To perform admissions and other oasis timepoints and revisits for clients requiring skilled nursing and fulfills the needs of patients in a variety of age groups, their family members, and the community in accordance with licensure, certification, training, and regulatory requirements. To respond effectively to new and changing working environments. To bear the responsibility and accountability for the nursing care that the patient receives while receiving services from Valley Home Care, Inc.
Experience
Computer and keyboard skills, Medical terminology required.
Special Skills/Qualifications
Current State of New Jersey license (or legal permission to work from the New Jersey Board of Nursing). Current and valid CPR certification. Demonstrates effective interaction and communication (oral, writing, presenting) skills. Ability to work accurately and pay attention to details, often changing from one task to another without loss of efficiency or composure. Ability to function competently in stressful situations and a changing work environment related to changing customer needs. Ability to work cooperatively within the health-system; with patients and family members; and with multidisciplinary team members (i.e., physicians, Rehab, etc.). Ability to utilize effective time management to set priorities, perform job related responsibilities, and respond quickly to emergency situations. Ability to use critical thinking and clinical reasoning skills to effectively problem-solve and deliver care.
*Driver`s License – current and valid driver`s license, registration, inspection sticker and appropriate level of insurance coverage required, to include collision. Employees must adhere to all procedural guidelines in HR Policy 108 including demonstrating an acceptable driving record for the duration of their employment as long as driving continues to be a responsibility of the job.
Education:
Graduate from an accredited School of Nursing. BSN preferred.
Competencies
1.Utilize assessment skills, evidence-based practice and data obtained through physical, psychosocial, cultural, spiritual, and educational assessments, interviews, monitoring devices, record reviews, clinical reports, etc., to complete an accurate and comprehensive assessment in an organized, systematic, thorough and accurate manner.
2.Recognizes deviations in client’s health status.
3.Collaborates with patient, family member(s), significant other and members of the health care team to develop patient and family centered plan of care.
4.Utilizes critical thinking and assessment data, knowledge of third party payor guidelines and understanding of regulatory requirement to interpret guidelines to the patient, offer alternatives if the payor guidelines do not meet the patient’s needs, and initiate individualized plans of care.
5.Utilizes clinical knowledge and critical thinking skills (problem solving, time management, priority setting, planning, and clinical reasoning) to safely provide direct patient care in a professional, caring, culturally-sensitive, and cost effective manner.
6.Uses supplies and resources in an efficient manner that minimizes waste.
7.Identifies outcomes and the patient’s response to treatment and procedures and the effectiveness of drug therapy (to include pain management initiatives). Reports all significant or unexpected outcomes to the patient’s physician.
8.Delegates patient-specific tasks and activities to the home health aide based on competencies, reguatory, Agency, and departmental policies, procedures, and fuidellines. Supervises and follows through with home health aides regarding delegated task and/or activity to ascertain if care was provided according to the care plan. Provides feedback on the HHA’s job performance to management.
9.Identifies the patient’s and/or family member’s/caregiver’s learning needs. Recognizes appropriate educational content and methodologies required to meet the patient’s and/or family member’s specific educational needs. Identifies the patient’s and/or family member’s/caregiver’s learning readiness. Initiates appropriate learning activities.
10.Accurately documents all assessment (including OASIS/SOC/Recertification visits) and reassessment data, patient care activities, interventions, patient’s responses, patient and family education, and discharge plans according to the plans of care and policies and procedures timely (i.e., dyspnea, wound).
11.Writes original and ongoing short orders timely to confirm physician’s orders that are consistent with other chart documents such as OASIS and covers all services and treatments provided. Submits within 24 hours of taking the verbal order.
12.Discharge records are completed in an accurate manner – (including all assessments/interventions and patient progress summaries) within 24 hours of the patient’s discharge.
13.Participates in the orientation and professional development of personnel and students as needed, requested or directed.
14.Attends and actively participates in at least 90% of all team and staff meetings.
15.Maintains personal growth and development activities to enhance clinical knowledge and professional skills (i.e., administration of medications, IV therapy venipuncture, treatments, procedures, etc.) to safely provide direct patient care in a professional, caring, culturally-sensitive, and cost effective manner.
16.Evaluates for age-specific behavior and initiates age specific care and safety measures.
17.Recognizes and responds to age-specific needs.
18.Effectively communicates information in an age specific manner to the patient, family member, physician, visitor, co-worker, etc.
19.Recognizes signs and symptoms of abuse or neglect.
