Job Title: Home Health Medical Social Worker (LCSW)

Job Overview:

We are actively seeking a compassionate and dedicated Medical Social Worker (MSW) to join our team. Responsibilities:

• Psychosocial Assessment:
• Conduct thorough psychosocial assessments to understand the emotional, social, and financial factors impacting patients’ health and recovery.
• Collaborate with the interdisciplinary team to develop and implement patient-centered care plans.
• Counseling and Support:
• Provide counseling and emotional support to patients and their families, addressing coping strategies and facilitating adjustment to health challenges.
• Assist in crisis intervention and bereavement support as needed.
• Community Resource Coordination:
• Identify and connect patients with appropriate community resources, support groups, and services to enhance their overall well-being.
• Advocate for patients to ensure they have access to necessary social services.
• Collaboration and Communication:
• Collaborate with healthcare professionals, including nurses, physicians, and therapists, to address patients’ holistic needs.
• Communicate effectively with patients, families, and the healthcare team to facilitate comprehensive care.
• Documentation:
• Maintain accurate and timely documentation of assessments, interventions, and coordination efforts in compliance with regulatory standards.
• Ensure meticulous record-keeping in the electronic health records (EHR) system.

Role Requirements:

• Education: Master of Social Work (MSW) degree from an accredited program.
• Licensing: Valid and active state licensure as a Clinical Social Worker (LCSW) is required.
• Experience: Previous experience in home health or a related healthcare setting is preferred.
• Compassion: Demonstrated empathy, sensitivity, and a patient-centered approach to social work.
• Communication Skills: Excellent verbal and written communication skills for effective interaction with patients, families, and the healthcare team.
• Resourcefulness: Ability to identify and coordinate community resources to meet the psychosocial needs of patients.
• Collaboration: Work collaboratively with the interdisciplinary team to ensure comprehensive patient care.
• Documentation Skills: Proficiency in maintaining detailed and accurate documentation in the electronic health records (EHR) system.
