Villa St. Francis is seeking a program manager for operational, clinical and life enrichment oversite for our 24 beds secured memory unit, Angela’s Crossing. The overall goal of Angela’s Crossing is to maximize our resident’s cognitive, psychosocial and functional skills in order to help maintain their highest level of functioning while offering a level of independence.
Angela’s Crossing at Villa St. Francis continues the ministry of Jesus by providing person centered services to meet the contemporary needs of aging individuals and their families. Responsibilities:
Promotes Villa St. Francis mission and values.
Ensures implementation of policies and procedures relating to Angela’s Crossing resident care.
Supervises all resident services, including the coordination of those provided by independently contracted ancillary health care providers as needed by residents and families.
Creates, schedules and improves resident life enrichment on Angela’s Crossing.
Oversees the care conference process for residents in Angela’s Crossing.
Coordinates and oversees monthly staff training regarding dementia and other cognitive impairments.
Advocates for residents to maintain their highest level of cognitive functioning.
Assures quality of resident services.
Assures the highest level of resident, family and employee satisfaction.
Promptly investigates complaints and reports findings and appropriate recommendations to the Executive Director and/or supervisor. Ensures follow-up communication occurs to complainant and that substantiated complaints are resolved to reasonable satisfaction of complainant.
Ensures compliance with all rules and regulations related to Resident care (i.e. HIPAA, State Regulations and OSHA, etc.).
Maintains a professional demeanor with all residents, their families, co-workers and the public.
Participates with the management team in the recruitment, interviewing, hiring, supervision, evaluation, and disciplinary process for resident care assistants.
Assists in orientation and training of community partners.
Assures resident care assistants adherence to all policies and procedures, work, safety, and other rules.
Administrates, coordinates, and directs all activities in accordance with policies and procedures.
Assures safety and security of community’s contents and inhabitants (residents, employees and visitors).
Assures proper use of equipment/supplies.
Participate with on call rotation for clinical operations.
Assures that adverse findings by company Quality reviews or state surveyors are promptly corrected and remain in compliance.
Performs other duties as assigned or required.
Qualifications:
RN or LPN
Must have at least one-year experience working in a similar role with responsibilities related to program operations for memory care/dementia residents.
Effective supervisory skills.
Ability to communicate and work with all levels of the Villa St. Francis and Felician Sister sponsorship.
Ability to recognize and communicate problems appropriately.
Ability to relate to the public, residents, families, employees, and other professionals appropriately.
Ability to maintain accurate records and provide information as requested and required.
Ability to communicate proficiently in English, verbally and in writing. Bilingual Spanish preferred.
Ability to work flexible hours to meet requirements of the job and be on call.
CPR certified.
Ability to teach/train others effectively.
