Job Discription
Position Summary: Manages and leads care plan for residents in collaboration with other departments Completes comprehensive resident assessments, resident cantered care plans, & functional screens. Provides ongoing care coordination and quality assurance monitoring of services. Maintains required documentation in adherence to established standards. Coordinates multidisciplinary programs & services within Angela’s Crossing. Communicates concerns to management and team members.
Essential Duties and Responsibilities: This list of duties and responsibilities is not all inclusive and may be expanded to include other duties and responsibilities as management may deem necessary from time to time.

· Extends excellent service to all customers
· Collaborates and provides leadership support to all departments and staff.
· Keeps current with Federal, State and local regulations and professional standards. Ensures compliance with all applicable regulations as they pertain to areas of responsibility.
· Assists residents in the completion of Power of Attorney for Healthcare paperwork. Recommends and aids in the activation of POA-HC as appropriate and completion of DNR as appropriate.
· Makes referrals for and facilitates guardianship and protective placement as appropriate.
· Makes referrals to other service providers and community resources as appropriate. Assists residents and family members in applying for and navigating the Family Care system.
· Coordinates and oversees the Individual Service plan process including but not limited to: scheduling and notifying appropriate participants of ISP meetings, developing appropriate goals and strategies to address the mental, emotional, and psychosocial needs of residents, and updating those not in attendance as appropriate.
· Coordinates and directs multidisciplinary approach for all programs and services for residents within Angela’s Crossing which involves RCA’s, Nutrition, Life Enrichment, Health Services and other related services to ensure continuity of care.
· Coordinates and oversees the process for Resident Satisfaction Surveys
· Serves as discharge coordinator assisting resident and family/legal representative with discharge planning and providing written discharge notification as required. Document discharge through completion of a discharge summary.
· Welcomes residents and assist resident/family members with the transition and adjustment to assisted living
· Informs residents of rights, responsibilities and rules governing their stay. Insures residents rights are protected at all times. Investigates and reports as required, alleged violations of resident rights.
· Completes a social history within 30 days of resident admission. Documents annual reviews/assessments, changes of condition and contacts with the resident/responsible party per DHS 83 regulations and professional standards.
· Assists in the development, implementation and revision of policies and procedures as requested.
· Maintains and tracks information related to quality assurance and decision indicators as assigned.
· Participates on committees as assigned
· Other duties as assigned.
Qualifications:
· Is able to advocate within the Resident First philosophy
· Bachelor or Master’s Degree in Social Work or closely-related field
· State Certification as a Social Worker preferred
· Working knowledge of the basic theory and principles of Social work
· Demonstrated ability to analyze an issue, identify options, and implement solutions
· Working knowledge of community health and social resources
· Knowledge of federal/state regulations and guidelines including resident rights and confidentiality
· Excellent oral and written communication skills
· The ability establish positive relationships with staff, residents, family members, and other professionals
· The ability and desire to provide professional and courteous service and care to geriatric residents
· Ability to develop and lead in-service training for facility staff, and educational sessions for residents, family members and the general community
Typical Schedule: Incumbent could be assigned to either five eight hour shifts per week, or three twelve hours shifts per week. Would be expected to be scheduled one weekend per month, and be on call one weekend per month.
