8-hour shift: 10:00 a.m. to 7:00 p.m., Monday through Friday, with rotating weekends

The Senior Clinical Nurse (RN) assesses, evaluates, and coordinates the health, wellness, and resident care of all residents while ensuring compliance with licensing and funding regulations.

Responsibilities

• Direct and oversee the care provided by Resident Care Assistants
• Manage the care aspects of a resident’s admission, orientation, and initial settling-in. 
• Lead the interdisciplinary care team in the development of each resident’ Individualized Service Plan (ISP) to promote his/her health, wellness, and quality of life
• Lead clinical quality and process improvement initiatives to mitigate risk and improve resident care outcomes
• Educate and supervise staff doing medication administration
• Collaborate with the social worker to facilitate a resident’s discharge when able to return home safely, or when his/her needs exceed CBRF licensure or actual staffing capabilities
• Maintain contact with physicians and other care providers, follow through on implementation of physician orders, and documentation of care/services provided
• Promote teamwork with all staff  to provide exceptional, ongoing health and wellness monitoring with early intervention to maintain each resident’s highest level of physical, social, and psychological well-being.
• Take turns serving as the on-call nurse on evenings, nights, and weekends

Qualifications:

• Current, unrestricted State of Wisconsin Registered Nurse (RN) license
• Current CPR certification
• 1 to 2 years of geriatric experience; preferably in an assisted living setting
• Excellent clinical knowledge and skills; skill in recognizing the physical, mental and emotional needs of residents; experience with developing care plans highly desirable.
• Demonstrated ability to analyze issues, identify options and implement solutions.
• Outstanding oral and written communication skills; ability to maintain professional and cooperative relationships with all customers—family, other professionals, residents and staff from all departments.
• Detail oriented leader with the ability to manage multiple priorities in a high pressure environment
• Familiarity with DHS 83 preferred
• Basic computer proficiency necessary to utilize email and resident care software.
