Chickahominy Health District is on the forefront of Public Health and Population Health and looking to hire an innovative, flexible and team focused, Public Health Nurses (PHN) to join our team. This exciting PHN position will focus on Family Planning, Maternal Child health, Sexually Transmitted Infections, Communicable Disease and Nursing Home Pre-Admission screenings, and Immunizations. This role will also involve community outreach and some evening and weekend hours. PHN will work alongside a dedicated team of other Chickahominy PHNs and health professionals.
This position promotes public health and well-being for residents primarily of Hanover County, however works in collaboration with Goochland, New Kent and Charles City. The PHN will assist in facilitating relationships among Chickahominy Health District, Community-based health care providers and community members and is knowledgeable of social and economic conditions related to public health and public health nursing. This PHN will work closely with the Department of Social Services to screen older adults and children with ongoing medical needs for eligibility of the Long Term Services and Supports (LTSS) waiver through Medicaid. Additional skilled services will include: assists/participates with communicable disease assuring appropriate patient intervention and follow-up, abnormal laboratory results and other conditions, and stay up to date on the most up-to-date immunization schedule to appropriately administer vaccinations. This nurse will promote awareness of the health department and other community resources and coordinate referrals to outside agencies and physicians.

This position will be based out of the Hanover Health Department, but travel to the other Health Departments within the district may be required. The Chickahominy Health District is comprised of Goochland, Hanover, New Kent and Charles City.
Employment is contingent upon satisfactory results of a state and federal criminal history background check and the Department of Social Service’s Child Abuse and Neglect Central Registry check, U.S. HHS IG Exclusion List check, employment reference check and E-Verify.
