Overview:
The role of Patient Home Program Manager is to manage the Discharge Lounge and Care Van operation. The purpose of the Patient Home program is to maintain safety and provide exceptional service to patients requiring assistance in moving from one location in the hospital to another, coordinate departure and transportation details, oversee the discharge lounge, and safely transport select patients to their homes via the Care Van. The Program Manager will lead and oversee the team that provides these services.
Responsibilities:
- Attract and retain impressive people providing exceptional service to all of Mass General Hospital
- Understand “Client’s Definition of Success,” deliver it and present it in very impressive manner at every opportunity.
- Proactively work to anticipate and deliver clients’ expectations.
- Work with the client to ensure ongoing hospital compliance of policies.
- Lead and manage all requirements of the patient to home program and ensure its success.
- Make patient service, care, and satisfaction the priority in every interaction by providing exceptional guest experiences.
- Interact with the patient to ensure that departure from the room is smooth and comfortable.
- Coordinate team schedule and departure details if being transported by the hospital team to discharge location.
- Interact with patient and valet staff to ensure the patient has a smooth transition to the vehicle.
- Look out for the safety and well-being of all team members, patients, and co-workers.
- Report all incidents in accordance with company policies.
- Assist in maintaining the cleanliness of the lounge.
- Ensure safe functioning of the Care Van at all times.
- Track data to share internally and with the client.
Qualifications:
Years of Experience: Experienced – supervisory healthcare preferred
Language Skills: Proficient in English both verbally and written
Education Requirements: High School Diploma/GED
