Wellpoint Care Network (formerly SaintA) – a Milwaukee Journal Sentinel Top Workplaces in 2021 and the 2021 BizTimes large non-profit of the year – is seeking a competent, compassionate, and career-oriented professional to serve as our ICT Service Facilitator. 

Job Purpose:
Our next ICT Service Facilitator will be responsible for the coordination of services for youth in the Comprehensive Community services program of Washington County. They will ensure needs of youth and family are met by providers to facilitate greater overall mental health and well-being.

Duties:

• Performs clinical assessments and develops and implements client treatment plans. Provides individual and family psychoeducation and skill development
• Develops a network of community support and resources, connects clients with resources as appropriate or makes referrals as needed.
• Establishes and maintains working knowledge of WCHSD programs, resources and supports.
• Facilitates team meetings.
• Maintains client files and participates in quarterly review of files. Monitors outcome data to measure program performance.

Qualifications:

• Bachelor’s degree in Social Work or equivalent required.
• Minimum of two (2) years’ work experience in child welfare or social service setting required.
• Proficient in the usage of computer software; demonstrate an understanding of computer file systems and computer software packages, e.g., Microsoft Office, Excel, Outlook, Teams, ADP, etc.
• Ability to work within a multi-disciplinary team.
• Ability to take initiative and act quickly and independently when the situation demands it; sees a need and takes appropriate action without being prompted or reminded; demonstrates ability to effectively deal with crisis situations.
• Ability to exhibit professionalism while working effectively and harmoniously with staff, program participants, social service agencies, legal parties, and the public.
• Demonstrate an ability to effective understanding and practice of cultural humility and ability work and communicate respectfully with individuals from diverse backgrounds.
• Valid Wisconsin driver’s license required.
• Physical exam, drug screen, motor vehicle report, and background checks are required for this position.

• Fast paced, flexible work schedule includes evenings and some weekends to accommodate program need. 
• Incumbents will occasionally need to be available for extra hours and/or provide flexibility to accommodate special events, peak demand or staff shortages.
