The Home Health Aide competently, reliably and safely provide in-home services including personal care, housekeeping, meal preparation and laundry, and companionship to Home Health Hospice clients according to a written Plan of Care.

ESSENTIAL FUNCTIONS: Home Health Aide

Follow the Plan of Care initiated by the RN/LPN or Director for each client. Activities and care may include, but are not limited to the following:

Personal Care Duties: Home Health Aide

1. Baths, showers, toileting, dressing/undressing, skin care, and other personal care needs.
2. Shopping and/or meal preparation.
3. Medication reminders.
4. Resident safety checks as assigned
5. Reporting changes in physical, mental or emotional condition to Wellness Nurse, Unit Manager, and/or WHCHS office
6. Documentation of visits and care given/refused and arrival/departure times as required.
7. Weight and vital signs as directed.
8. Assist with gait, including proper use of walkers or other equipment.
9. Position in bed, assist with ROM or other routine exercise programs as directed.
10. Provide all tasks within licensed scope of practice in a home setting.
11. Provide foot/nail care as instructed by supervisors and per Plan of Care. l.
12. Follow universal precautions.
13. Knowledgeable of and follows emergency procedures.

Housekeeping Duties

1. Assist in maintaining neat, orderly, and safe environment in each client’s home.
2. Housekeeping, laundry, meal preparation, grocery shopping, errands as requested by client and included in Plan of Care.
3. Maintain a written log with receipts of all client money used to purchase goods or materials for client’s care.
4. Performs other duties as requested and as indicated on Plan of Care.

Education Background

1. High school diploma or equivalent.
2. Completion with a passing grade of an NAC training program.

Licensure, Registration, Certification

1. Possess and maintain Washington state NAC or HCA (Home Care Aide) license.
2. Maintain annual requirements for TB, CPR/First Aid, and continuing education.
3. Have a valid Washington State Driver’s license or have access to public transportation unless assignments can be made within transportation limits.
4. Ability to safely drive a car if applicable and provide proof of current automobile insurance.

Special Training/Skills

1. Recognize and report adverse changes to RN/LPN – Wellness Nurse (campus) or Home Health office (after hours for Wellness Nurse and for all off-campus clients).
2. Must be able to read and write in English and to communicate with clients / patients / staff / coworkers / agency staff.
3. Must be able to remain calm under stress.
4. Coordinate multiple tasks simultaneously.
5. Understand and respond to a diverse population.

Physical Demands

1. Ability to give all direct client care within scope of license and according to written Plan of Care.
2. Ability to move about in congested areas.
3. Freedom from communicable infection(s).
4. Ability to walk, bend, stoop and squat for client care and assessment.
5. Sit for charting, phone use, feeding of residents and computer use.
6. Ability to lift up to 50 lbs.
