SUMMARYUnder the direction of the Director, Clinical Integration & Population Health, this position has responsibility and accountability for successfully developing, managing, and overseeing front line operations for population health programs across YNHHS and the wider network. The role includes program development, implementation and evaluation. The role involves substantial collaboration with leaders and internal and external stakeholders to facilitate the coordination and delivery of healthcare services, development of ambulatory quality improvement programs, and support the transition to value based care to enhance the health of the communities we serve.QUALIFICATIONS1. Directly manages a team of population health consultants and other staff as programs develop.

• 1.1. Actively participates in and supports the development, mentoring and coaching of staff.
• 1.2. Understands YNHHS vision, goals and strategies and communicates to others. Able to set clear goals and directions.
• 1.3. Provides operational leadership for population health programs and staff, while managing projects as indicated by the leadership team
• 1.4. Develops population health improvement methodologies and competencies to serve as framework for team development and organizational understanding of population health

2. Develops and executes strategies and programs that advance the organization’s development of population health services critical to the successful provision of value-based care.

• 2.1. Develops and supports the population health governance and committee infrastructure
• 2.2. In conjunction with clinical and administrative leaders, develops key priorities for advancement of population health management using the principles of clinical Integration, care transformation, and clinical quality improvement
• 2.3. Develops and oversees implementation plan and coordination of resources to execute the plan
• 2.4. Communicates progress and impact to key stakeholders and senior leaders
• 2.5. Supports strategic and operational development of facility and provider networks, including employed, faculty, and independent clinicians who share our values, in service of high performance in value-based arrangements

3. Leads the development, execution and resourcing of playbooks and action plans aimed at supporting a scalable model for operational success with value-based care.

• 3.1. Develops expertise in the areas of clinical integration standards, evidence based ambulatory quality measures, social determinants of health and population health data aimed at establishing, measuring and monitoring a high standard of clinical excellence for our patients.
• 3.2. Conducts benchmarking and best practice research into models for team based care, clinical decision support, and workflow enhancements; develops proposals and business plans for high-impact opportunities.
• 3.3. In partnership with the population health leadership team, Finance and ITS, develops the reporting and analytics infrastructure to evaluate the health of the population(s), measure program performance and identify areas for improvement.
• 3.4. Develops strategies, tactics, and tools for identifying practices or populations with the greatest opportunity for performance improvement and managing the deployment of resources to spread improvements.
• 3.5. Oversees the clinician engagement strategies for the successful deployment, spread and adoption of population health programs and initiatives
• 3.6. Oversees the development and execution of patient engagement strategies, in coordination with the system Patient Experience team, aligned with the goals of improving population health and advancing the system signature of care.

4. Evaluates results and value delivered by population health programs.

• 4.1. Identifies and develops opportunities to assess population health measures across stakeholders
• 4.2. Facilitates alignment of population health programs and measures across YNHHS internal and external stakeholders where possible and supports all stakeholders in attaining improvement to goals.
• 4.3. Works closely and in collaboration with leadership in many YNHHS and other stakeholder’s functional areas, including clinical practices and service lines, YNHHS delivery networks, performance improvement, patient experience, marketing and communications, internal and external partners to set the strategic direction for program development, execution and resource deployment

EDUCATIONMasters degree in business, public health, nursing, healthcare administration or other related field required. Bachelor’s degree required. Clinical background is preferred. Advanced training in project management, performance improvement and quality improvement methodologies is strongly preferred.EXPERIENCE7-10 years of progressive experience required with increasing responsibilities and experience in managing complex teams and programs with demonstrated successful outcomes.
