Job Description

The Mental Health Practitioner will provide services in adult rehabilitative mental health services (ARMHS) and crisis stabilization programs. In ARMHS, these services are rehabilitative and enable the recipient to develop and enhance psychiatric stability, social competencies, personal and emotional adjustment, and independent living and community skills, when these abilities are impaired by the symptoms of mental illness. Crisis stabilization services are a short-term service where the staff member will meet, typically, once per week with the client and aide in connecting to needed supports and collaborating with existing supports.
Responsibilities

• Maintain knowledge of city and/or county mental health, medical, financial, housing and other human service agencies.
• Maintain a caseload of clients.
• Complete admission paperwork within designated timeframes.
• Complete discharge summary within 30 days of closure.
• Facilitate or co-facilitate rehabilitative independent living skill classes.
• Provide ARMHS services to clients as often as needed, a minimum of once per week.
• Request, complete and update all necessary documentation. Maintain the medical record and comply with all HIPAA and confidentiality requirements.
• Work collaboratively with all members of the treatment team, via on-going written and verbal communication. Coordinate treatment planning meetings including the client, team members and natural supports.
• Understand and ensure proper billing codes for services. Ensure billing occurs according to set timeframes.
• Carry out care coordination activities within designated timeframes.

Qualifications

• Bachelor degree in Social Work or a behavioral science field and 2,000 hours of experience required
• Must meet the State of MN’s definition of a Mental Health Practitioner
• Knowledge of community resources and crisis services preferred
