Community Health Educator will provide support, counseling and education to patients to maintain health, slow progression of chronic disease, reduce complications and improve patient empowerment. CHE will support the delivery of quality health care and the elimination of health disparities among targeted populations

Provide pre and post test HIV counseling and education to clients of Baystate Emergency Department and the community at large. Outreach & organizing support at community events and related duties as assigned. Incumbent will support the delivery of quality health care and the elimination of health disparities among targeted populations.

• Conduct Counseling & Testing in a Community Health Center

• Conduct community outreach & organizing

• Provide HIV/AIDS education; Understand harm reduction as it relates to HIV, Viral Hepatitus & STI

• Community oriented; knowledge of local service networks and available resources in the community

• Maintain & submit complete & accurate records, data sheets, confidential forms, & blood submission forms

• Schedule appointments & conduct walk-in sessions

• Perform finger sticks and veinipuntures

Required Experience:

• High School or GED

• Registered Medical Assistant/ Certified Medical Assistant Required
• CPR Certified

• Certified Community Health Educator preferred

• 1 year phlebotomy experience required

• 2 years community experience in HIV/AIDS prevention & education as well as 2 experience in community outreach & organizing

• Computer literacy, strong organizational & record keeping,

• Bilingual written & verbal preferred -Demonstrated cultural competency with low-income, intravenous drug users and African American & Latino target populations
