Sovah Health Danville is seeking a professional team player to serve as a hospital-based Community Health Worker (CHW) to service the Dan River region area. This position will partner with care teams within the health system to ensure the most comprehensive care for patients in need. As a member of the care team, the CHW will take part in case review meetings and rounds, and support patient care planning.
The CHW will also use external and community networks/resources to connect patients to sources of support to improve patient care, outcomes, and connect patients to a primary care provider post discharge. The hospital based CHW will promote better health through encouraging behavior change, and will be trained to use motivational interviewing and other coaching techniques to motivate and encourage patients to make changes to improve their own health.
Establishing trusting relationships with clients and their families while providing general support and encouragement.
Maintaining a high level of confidentiality and integrity.
Conducting intake interviews with clients.
Coaching clients in effective management of their chronic health conditions and self-care while motivate clients to be active, engaged participants in their health.
Guide clients according to clinical practice guidelines and best practices for their disease.
Helping clients set personal goals and develop health/care management plans.
Assisting clients in understanding care plans and instructions.
Providing continuous follow-up with clients via phone calls, home visits and visits to other settings where clients can be found, from initial identification through closure.
Assisting clients in accessing health-related services, including but not limited to: obtaining a primary care provider, providing instruction on appropriate use of a primary care provider, overcoming barriers to obtaining needed medical care and social services.
Facilitating communication and client empowerment in interactions with healthcare and social services.
Assisting clients with completing relevant applications and registration/enrollment forms.
Providing referrals for services to community agencies as appropriate.
Helping clients connect with transportation resources and give appointment reminders in special circumstances.
Acting as a client advocate and liaison between the client/family and community service agencies (i.e. schools, Department Human Services, hospitals, support groups, etc.), facilitating communication and coordination of services between providers.
Provide ongoing follow up support and services to graduated clients, as needed.
Minimum Education
High School diploma or equivalent required.

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Organization skills and confidentiality a must. Must function independently, is flexible and able to relate to ill, disabled, elderly and emotionally upset internal and external customers. The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served. The growth and development of the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirement relative to his or her age, specific needs and to provide the care needed as described in the departmental policies and procedures
Minimum Work Experience
Minimum of 2 years of experience in a healthcare, community-based, or social work environment or any combination of experience which would provide an equivalent background.
