Community Health Worker – Spokane Region (Ferry, Stevens, Pend Oreille, Lincoln, Spokane, Adams)This position is remote but requires travel within the Spokane Region (Ferry, Stevens, Pend Oreille, Lincoln, Spokane and Adams counties)
Job Summary
The Community Health Worker serves as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. They also build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
Essential Functions

• Responsible for engaging prospective care management program participants who have been identified through data analysis and referral sources to likely benefit from care management program offerings
• Utilizes Internet databases, health record systems, and other resources to locate potential eligible Member contact information
• Find members that CHPW has been unable to reach by phone. This includes but is not limited to, going out into the community to find members, connecting with collateral contacts in the health care and social services systems, reviewing pharmacy information, attempting to locate member at last known address
• Conducts outreach to identified clients by telephone, mail, and/or hospital or home visit
• Provides follow up services via telephonic or face to face engagement with clients and service planning partners as needed to coordinate reminder calls, medication and medical appointments, upon request from the care management team
• Consults with care managers in securing and identifying needed referrals to community and network medical, behavioral health and social assistance providers through telephonic and/or face to face outreach
• Provides community outreach services including home visits, assisting individuals with accessing transportation services, educating enrollees on healthy behaviors, and providing information on community resources
• Provides scheduled activities that promote socialization, recovery, self-advocacy, development of natural supports, and maintenance of community living skills
• Provides information to increase the enrollee’s knowledge about his or her health conditions and improve adherence to prescribed treatment
• Ensures that the Supervisor of Community Programs is made aware of any potential risk management issues in a timely manner
• Prepares for and participates in ongoing peer case staffing for all active cases on a regular basis
• Provides oral and/or written status updates regarding client alerts, progress and needs to responsible care managers and providers, legal mandate, or other care plan affiliates as needed to assist the program and enrollees. Ensures case documentation is consistent with policies and procedures
• Acts as an internal consultant to other departments for Community Programs and peer support
• This position requires traveling on behalf of the Company and working in the field at least 50% of the time. It is essential that a current driver’s license, proof of insurance and an acceptable driving record are maintained
• Non-Exempt – Reporting to work on time and for all scheduled shifts is essential to this position.

Qualifications
Education

• High school diploma or equivalent required; Bachelor’s Degree preferred.

License, Certifications, and other Specialized Training

• Certification as Peer Specialist may substitute for minimum experience and education requirements.
• Valid state issued drivers’ license required

Experience

• Three (3) years of job-related experience providing medical, mental health or substance abuse-focused services to individuals with chronic medical conditions and/or severe and persistent mental illness.
• Experience and familiarity with at-risk populations, communities of color and local resources for persons with behavioral and/or physical health needs.

• Complete and successfully pass a criminal background check
• Has not been sanctioned or excluded from participation in federal or state healthcare programs by a federal or state law enforcement, regulatory, or licensing agency

SENSORY/PHYSICAL/MENTAL REQUIREMENTS
Sensory*:

• Speaking, hearing, near vision, far vision, depth perception, peripheral vision, touch, smell, and balance.

Physical*:

• Extended periods of sitting, computer use, talking and possibly standing
• Simple grasp, firm grasp, fine manipulation, pinch, finger dexterity, supination/pronation, wrist flexion
• Frequent torso/back static position; occasional stooping, bending and twisting
• Some kneeling, pushing, pulling, lifting and carrying (not over 25 pounds), twisting and reaching

Mental:

• Frequent decision-making. Ability to gather and assess data, determine appropriate actions, apply protocols and knowledge to unique situations, problem-solve and provide consultation.

WORK ENVIRONMENT
Office environment with frequent environmental exposure to low-grade radiation from computer monitors; fast paced with frequent interruptions.
