Become a part of our team of competent and compassionate professionals as a Community Health Worker (CHW) in the Holyoke F.I.T (Families in Transition) grant program at Behavioral Health Network! Holyoke F.I.T (Families in Transition) is a grant funded program designed to provide wrap-around, outreach, team based care to eligible families through Holyoke Public Schools. The goal of this program is to help families in need achieve a greater level of health, wellness and stability. We provide supportive outreach services to assist families in navigating difficult systems and in connecting with relevant services in the community.
The Holyoke F.I.T (Families in Transition) grant program is an innovative model of care, working with families, Holyoke Public Schools and community providers to address the whole health needs of the families in our culturally diverse community. Holyoke F.I.T (Families in Transition) is a family and person-centered model which focuses on collaborative goal setting to address unmet housing insecurities, medical, behavioral mental and social needs. Our team consists of a Clinical Care Manager (CCM) and Community Health Worker (CHW) who will meet together with families to assess their strengths and needs and develop with the family a Family Care Plan to address needs and encourage strengths. Families will receive guidance and support from CCM and CHW on implementing the Family Care plan which will include identifying and maximizing connections with other community resources and supports. Examples of community resources and supports include:
