Minimum Qualifications
Bachelor’s degree in a related field and two years of experience providing direct patient/client services in a health, human service or community based organization; OR an Associate’s degree in a related field and four years of such experience; OR six years of such experience. A Master’s degree in a related field may substitute for one year of such experience.
This position requires that the incumbent will report to the official work location and live within commuting distance to the official work location. Telecommuting may not be available. If telecommuting is available, the incumbent will be required to have a telephone/mobile device, and internet access.

The incumbent(s) hired into this position must live within commuting distance to the official work location. Successful completion of the Public Health Essentials certificate program will be required.
Preferred Qualifications
Bachelor’s degree in a related field. Experience working with community based organizations or health care providers; experience managing or developing and implementing public health program services; experience in the provision of customer service; demonstrated experience with data analysis for quality improvement; experience working with the long term care population; and proficiency with Microsoft Office product suite (i.e., Word, SharePoint, MSTeams, Excel).
Responsibilities
The Public Health Representative III will work within a surveillance team consisting of various disciplines to perform inspections of adult care facilities based on complaints and/or incident and/or death reports received, recertification, follow-up, infection control, and pre-licensure/certification. The incumbent will draft inspection reports and perform quality assurance reviews of the reports prior to issuance. The incumbent will review facility-submitted plans of correction to assess for regulatory compliance, and perform outreach and develop educational forums for adult care facilities as it relates to COVID-19 infection control practices based on data collected through the survey process thereby assisting in the State’s recovery from the pandemic. The incumbent will perform other appropriate related duties.
The position is supported by the New York State Public Health Corps Fellowship Program.
Successful completion of the Public Health Essentials certificate program will be required.
Travel, 25%-50% of the time, will be required.
