Bethany Behavioral, a Unit of Cedar Ridge, is a 60-bed psychiatric hospital offering adult behavioral health programs and treatment services. Our team of dedicated professionals provides crisis stabilization and medication management to patients who have disorders such as depression, anxiety, mood disorders, paranoia and schizophrenia. Position Summary:
The Step-Down Navigator/Case Manager is responsible for meeting with all eligible adult patients at Cedar Ridge at Bethany to discuss the benefits of stepdown services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership and associated departments and will be responsible for routinely collecting and entering data, specific to eligibility, into Midas for trending report purposes. The navigator will have a thorough understanding of the step-down process and will coordinate between departments. Assists therapists with discharge process, develops referral sources, and performs duties in accordance with the ANA Standards of Practice for Psychiatric Mental Health Care, and facility mission, values, policies, and behavioral standards.
