Affinity Health Center is seeking a full time Bilingual Community Health Worker. The Community Health Worker will seek to improve access to health care services for people living with diabetes or those at risk for diabetes. CHW will seek to bridge any gaps between community members and the health and social service system to enhance communication between people living with diabetes and those providing diabetes care. CHW will support patient and community access to education and care, helping to reduce health disparities in underserved communities.Requirements:
