Job Description
Summary: Attentive home health LPN sought to visit patients in their homes around the Gurnee/ Waukegan area of IL. Provide nursing care under direction of RNs and in compliance with the physicians’ orders.Flexible schedule, take patients base on your availability, close to home, and one on one care.Responsibilities: – Patient assessment and observation, administration and monitoring of prescribed medication, treatments and therapies in patient’s home environment- Follow designed Plan of Care- Communication with other members of the patients’ multi-disciplinary team- Prepare clinical notes and update primary Physician- Health promotion and teachingRequirements: – Graduate of an accredited school of nursing; LPN Program- Current and unrestricted licensure in IL- Valid driver’s license and auto insurance- Health/work clearance letter signed by MD- CPR certificated- Be able to work and travel independently- Competency with a wide assortment of nursing skills such as foley insertion/care, wound dressings, bowel care, endocrine and GI disease management, etc.- Knowledge of basic computer operations such as typing, Windows OS, internet, and email- Excellent observation, verbal, and written skills- At least 3 year of clinical patient care experience in a hospital or other clinical setting- Preferred at least 1 year of prior home health visiting experience- Preferred experience in any home health software (EMR) such as Kinnser/Wellsky, Axxess, DataSoft Logic, Homecare Homebase, etcApply at: https://alternativehhcare.aidaform.com/nurse-applyJob Types: Part-time, PRN, Per diem, Travel nursingPay: $50.00 per hourBenefits:
