Job Summary
The Adult BH Services Case Manager assists with accomplishing patient goals by working cooperatively with members of patients’ BH care team, assisting with care referrals/recommendations, and acting as a liaison for psychiatry and therapy services through GLBHC. Participates in a team-based approach to care in the Patient Centered Medical Home (PCMH). Provides support and coordination to optimize behavioral health clinical services. Participates in activities related to utilization, performance and quality improvement.
- Assists with administrative support, care coordination, and patient outreach functions of clinical services as assigned.
- Assists with data collection, compiling reports, panel management and database management for Adult BH Services necessary to optimize patient care.
- Conducts chart audits for content, compliance & quality, and other utilization priorities as assigned.
- Coordinates associated tracking. Manages Referral Desktop in EMR as assigned.
- Crisis Call triage. Follows clinical guidelines to assess and provide needed intervention and to link patients to appropriate resources.
Job Requirements
- Bachelors degree in social work, psychology or similar human services field from an accredited school.
- LBSW, LLBSW or SST certification/licensure preferred.
- Minimum of one to two (1-2) years experience in the behavioral health setting.
- Ability to communicate with diverse populations. Able to coordinate patient services and deal effectively with other service organizations. Skilled in written and oral communication. Able to demonstrate positive customer service skills.
