Job Title: Care Consultant – DCC Program – Washington State Chapter
Location: Lynnwood, Tukwila, or Spokane WA
Full time based on 37.5 hours per week

Reports To: Senior Clinical Manager

Position Summary:
The Care Consultant provides consultation services to persons affected by Alzheimer’s disease and other dementia and their caregivers referred under the Knight Family Dementia Care Coordination (DCC) program. The care consultant will deliver consultations by phone or videoconference. This is a full time temporary grant funded position.

Essential functions and responsibilities include, but are not limited to:
TEAM PARTICIPATION
