Complete assessments and provide case management services to persons 60 years of age and older residing in Sedgwick, Butler and Harvey Counties requesting Senior Care Act (SCA), In-Home services.
Complete assessment and reassessments for clients referred to and/or active on the SCA program.
Provide Case Management to ensure clients are receiving appropriate care, services and support throughout the duration on the program.
Complete client service activities, documentation, and record management in compliance with KDADS policy guidelines.
Minimum Qualifications: Bachelor’s degree from an accredited college or university in Social Work, Gerontology, Nursing, Counseling, Family Studies, or Human Development. Minimum of one (1) year experience in the geriatric services and/or disabilities field. Minimum of one (1) year of Case Management experience. Valid State of Kansas driver s license and proof of automobile insurance.
Preferred Qualifications: Master’s degree from an accredited college or university in Social Work, Gerontology, Nursing, or Counseling. Two (2) years’ experience in the field of aging or an Area Agency on Aging. Bilingual skills a plus.
