Constellation is hiring for Per Diem Home Health and Hospice Aides- Must be CNA certified!

Schedule: – Week Day schedules, Saturday & Sunday hours availability, first and second shift.

Territory: Seeking to hire in the following cities & Towns: Augusta, Bangor, Brewer, Kennebunk, Portland and York County
Qualifications:

Must have your CNA certification for the state of Maine. Completion of a (minimum) seventy-five-hour training program for hospice aides that has been approved by the Department of Public Health; and/or demonstrated competency as documented through a passing grade on a competency evaluation; a caring attitude toward the sick, disabled and elderly; the ability to read and write; satisfactory rating on annual performance evaluation. If a Hospice Aide is on the State Registry they must be listed in good standing. Absent criminal background check. Understands the Home Care and Hospice Philosophies. The Hospice Aide will be trained in appropriate and safe techniques for performing personal hygiene and grooming tasks that include bed bath, sponge bath, and shower bath, and hair shampooing in sink, tub, and bed. The Hospice Adie will also be trained in recognizing and reporting changes in skin condition.
Job Summary:

The primary function of a home health aide is to assist with personal care of the patient. The services of a home health aide are ordered by a physician and are given under the supervision of a registered professional nurse or therapist. The specific personal care for each patient will be determined by the registered nurse or therapist and will be given in writing to the home health aide. Aides will be available for service during agency usual hours of operation. On occasion, the agency may assign home health aides to evening and night shifts.

Responsibilities:

• Assist the patient with personal care

• • Bed bath, Sponge, tub, and shower bath
  • Care of the hair, skin, teeth, and nails

• Includes shampoo in sink, tub and bed if directed by the registered nurse

• Clean nails and use emery board. No cutting or clipping of nails is permitted

• Shave patients with an electric razor

• Assist the patient with activities of daily living

• • Assist with dressing
  • Assist with eating
  • Assist to bathroom, or with use of urinal, bedpan or commode
  • Assist ambulation with cane, walker or wheelchair
  • Reposition/turn patient in bed

• Assist with prescribed range of motion exercises and specific exercises after instruction from professional health personnel.

• Measure and record intake and output when directed by the registered nurse

• Prepare meals and special diets as directed. May assist with meal planning and limited grocery shopping

• Perform light housekeeping duties necessary to maintain a safe environment to prevent or postpone institutionalization, including:

• Change bed linen

• Light cleaning of patient’s living area

• Laundering essential to the comfort and cleanliness of patient

• Maintain a record of activities and submit this record to the agency once a week

• The Home Health aide is a member of the interdisciplinary team and is trained to recognize and report changes in: skin condition, behavior or overall condition of the patient to the Rn or other appropriate skilled agency professional.

• Completes appropriate records in compliance with the Agencies policies and procedures

• Attend at least 12 hours of in service training per year, for hospice inclusive of 6 hours of hospice specific education.

• Maintain confidentiality in all matters related to patient care

• Reminds patients to take medications which are ordinarily self-administered, as instructed by the supervising nurse

• Report to the supervising R.N. or therapist concerning all matters related to patient care

• Participate in case conferences

• To perform all required services consistently as ordered

• To be observed during the care provision process by a designated supervisor every six months

• A Home Health Aide will be required to go through retraining if there has been no formal work in 24 months.

• Will perform other duties as assigned.

• Occasional weekend coverage & holidays (as needed)

Restrictions:

• Must not give home or cell phone number to patients

• May not accept outside employment with patients served by Constellation Health Services

• Must not discuss own personal problems with the patient

• May not accept payment or tips

• May not buy any alcoholic beverages for the patient or the patient’s family

• May not smoke in the patient’s home

Essential Capabilities:

• Able to read printed word

• Able to communicate effectively in English-verbal and written

• Able to make own arrangements for transportation to patient’s home either through public transportation or private vehicle

• Maturity and sensitivity in dealing with emotional or stressful patient situations, such as death, pain, or grieving

• Physically capable of performing the job

Physical Requirements:

• Bending – Frequent bending is required to care for patients in their bed or chair

• Lifting – Lifting of patients may be required in transfer from bed to chair, chair to bed, up in chair or bed. Also other lifting of objects may be required. Must be able to lift 50 pounds and in an emergency situation to handle a patient’s full body weight.

• Walking – is required for entry and mobility through the patient’s home

• Climbing – stair climbing is required in many apartments and homes which may have several levels of stairs

• 5. Crawling – may be required while assisting patients with personal care or performing household duties

• 6. Touch – the employee must be able to distinguish hot and cold temperatures. The employee must have fine motor skills to grasp items, manual dexterity, and the motor skill to lift items.

• Smell – the employee must be able to detect odors and distinguish odors

• Vision – normal range of vision is required with or without correction

• Verbal – the employee must be able to speak clearly when forming words and changing vocal tone to accommodate to clients’ needs

• Hearing – normal range of hearing is required with or without correction

Exposure Hazards:

The following hazards may vary for each case assignment.

• Temperature: Household temperature can vary in extremes of hot to cold

• Fumes/Smoke: May be present in households

• Noise: Noise levels can vary in homes from loud to soft. Also external and environmental noise may be present.

• Dust: Patient households may have dust in varying degrees

• Animals: Patients may have household pets.
