Corrections Specialist 2 – Patient Care Navigator (SCCC) Full Time

• Coordinate, facilitate and negotiate services and resources to support coordination and continuity of care.
• Collaborate with physicians, peers, community colleagues and others to coordinate care across the health care continuum.
• Continually improve patient care and services.
• Work to eliminate barriers that might otherwise adversely impact patient care/outcomes.
• Access and act on information related to denials and barriers to ensure effective continuity of care.
• Refer quality, infection control and risk management issues to appropriate individual or department.
• Work with patients and refer complex cases to the primary care team to facilitate timely multidisciplinary case conferences.
• Provide liaison services between provider and clients.
• Assist patients along their care pathway by serving as an advocate.
• Provide education and logistical support to improve patient preparedness for treatment and to streamline care path transitions.
• Collaborate with the patient, patient representatives and interdisciplinary team to facilitate optimal outcomes as the patient transitions though the healthcare continuum.
• Provide support for patients after hospitalization or infirmary stay to promote self care.
• Maintain current knowledge of departmental policies/standards/procedures, new clinical procedures/processes, patient safety goals, core measure quality indicators and other issues/requirements impacting matters within assigned scope of responsibility.
• Participate as successful member of a multi-disciplinary team, understanding the value of individual work styles, roles and standards.
• Basic Knowledge of appropriate boundaries and limits around personal self-disclosure when working as a member of health and support services.
• Communicate effectively with other members of a diverse care team using appropriate interpersonal skills, group facilitation a conflict management skills as appropriate.
• Sensitivity to coordination of care requirements for all patients from a variety of ethnic, cultural, social, and economic backgrounds with varied medical and developmental needs.
• Assist clients in achieving identified health improvement and stabilization goals.
• Ensure that all documents and reports are compliant with regulatory requirements and meet HIPPA and health plan standards for content and timeliness.
• Maintain confidentiality and exercises discretion at all times.
• Document encounters in the medical record with clarity and conciseness unique to each specific patient or family interaction and in accordance with professional, legal, regulatory and departmental standards.

Required Qualifications:

• Satisfactory completion of two (2) years college from an accredited college or university whose accreditation is recognized by the US Department of Education and the Council for Higher Education Accreditation (CHEA) and two (2) years of experience in a health care setting.
• Proficiency using Microsoft Office software (Word, Excel, Outlook).
• Excellent written and verbal communication skills.

• Salary is based on experience and is determined at the time a final offer of employment is made. The salary range listed in the job posting includes Steps A-M. Step M is a longevity step and is typically gained six years after being assigned to Step L in your permanent salary range.
