Crescendo MD is seeking a Patient Care Advocate for our full-service private practice in Portola Valley, CA. Our Patient Care Advocates (PCAs) work closely with our entire team and are integral to our practice operation and patient experience.

The PCA role is primary for delivering care-coordination services for our patients and doctor teams, cultivating personal relationships with our patients, properly organizing and prioritizing projects, and staying two steps ahead of any issues.

Responsibilities and Duties:

Provide care coordination for Crescendo MD patients, including

• Serve as the main point of contact for patients with the front office
• Interface with other offices and hospitals on behalf of patients
• Schedule appointments for all types of visits for MD and RNs: house calls, office visits, referrals, labs imaging, etc.
• Track outside appointments
• Track lab results, consult notes and other patient records, organize and upload to chart and ensure the medical team has prompt access
• Facilitate any patient needs
• Answer, screen, triage, and transfer phone calls coming to the main office line during office hours
• Organize and forward summaries of results and outside visit notes to MDs and RNs as soon as available
• Contribute to team projects to help improve and build our organization

Qualifications:

• Exceptional professionalism in all personal, telephone, and written interactions with a strong understanding of confidentiality and discretion
• Bachelor’s Degree preferred
• Experience in a medical office setting preferred
• Medical Assisting license preferred
• High energy and enthusiasm, including a passion for our mission
• Strong drive for continued self and group improvement
• Computer literacy and high comfort level working across multiple digital platforms- Apple, G Suite, Electronic Medical Records, etc
• Adaptability, as roles and responsibilities will change as the practice thrives and develops further

Job Type: Full-time

Pay: $23.00 – $35.00 per hour
