Job Description:
Provide care for our patients, by supporting, comforting, and managing symptoms.
Create a personalized plan of care in collaboration with physicians and the interdisciplinary team.
Educate our patients and their families on next steps in in the disease process including self-management.
Coordinate and perform medical treatments to enhance the well-being of the patient with the goal of maintaining comfort and symptom management.
Perform all duties with a heart for hospice, and a passion for compassion.
Qualifications:
Valid registered nursing license (required)
1+ years of registered nursing experience
Associate’s Degree in Nursing required
You have a genuine desire to care for those facing end of life
