POSITION SUMMARY:
This position provides information, linkage to resources, and assistance with access to services for persons with Dementia and their caregivers. This position is also responsible to provide education and awareness to the community on Dementia.
ESSENTIAL FUNCTIONS:
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. To perform this job successfully, an individual must be able to perform each duty satisfactorily. Other duties may be required and assigned.
• Develops and maintains collaborative relationships with physicians, dementia diagnostic clinics and other health and long-term care providers for purposes of referral and service improvement.
• Delivers person-centered service to those diagnosed or requesting cognitive assessments: cognitive screens, follow-up after doctor appointment/diagnosis, connection to resources, short term service coordination, assists with planning legal, financial and engagement activities. Follows-up with referring physician.
• Schedules and completes community outreach and awareness to professionals, employers, civic and faith groups educating about dementia, communication and community resources. Creates presentations for community groups with information about DCS services, dementia, communication, caregiving, planning and community resources including the ADRC services.
• Initiates and maintains relationships with emergency responders, healthcare providers to educate, for referral exchange, and support to better service delivery to those with dementia in communities.
• Recruits consumers for evidence-based programs to provide education/support through Memory Care Connections for the caregivers and LEEPS for the person with the diagnosis. Recruits, trains, and monitors volunteers to deliver LEEPS.
• Recruits consumers for the DICE program to help caregivers understand disease induced behaviors.
• Participates actively in DCS statewide/regional meetings, conference calls, training programs. Collaborates with fellow DCS on program and training development and service delivery. Mentors new DCS staff. Provides education to colleagues on program and outcomes. Contributes to program development and evaluation.
• Records/reports data into electronic recording systems required by the State of Wisconsin for monthly reports, completes written reports as required by the State of Wisconsin.
• Collaborates with coalition members multiple communities to advance dementia friendly communities, training for businesses and emergency responders, and support for caregivers and persons with dementia.
• Provides training to businesses flexing with their schedule and offering as many as needed for their shifts/staff.
• Develops and directs Memory Cafes in county recruiting volunteer facilitators as needed and providing direction, supervision, and program content/speakers. Collaborates with community members to host sites/refreshments.
• Creates and maintains active relationships with media outlets to advance public awareness and knowledge about dementia, dementia care, services, resources, the ADRC.
• Other duties as assigned.

• Thorough knowledge in the areas of health care financing, income maintenance, community based services, consumer finance and protective services and probate.
• Thorough knowledge of community and agency resources and programs services.
• Advanced knowledge of interviewing and supportive counseling techniques including family meetings.
• General knowledge of social casework techniques.
• General knowledge of human development and behavior.
• Ability to recruit, plan and supervise the work of staff and volunteers.
• Ability to deal effectively with older adults and the legal system.
• Ability to analyze facts and exercise sound judgment in arriving at conclusions.
• Ability to communicate complex ideas effectively, orally and in writing.
• Ability to prepare clear and concise reports.
• Ability to establish and maintain effective working relationships with clients, associates and the general public.
• General knowledge of standard office procedures, practices, equipment and office assistance techniques.
• Must be proficient in workplace English and spelling.
• Possess accurate basic math skills.
• Knowledge of the County and department programs and policies.
• Some knowledge of the use of a multi-line telephone system.
• Ability to read and understand basic County and State policies and procedure of limited scope and difficulty.
• Ability to type accurately at a reasonable rate of speed.
• Ability to operate standard office equipment and perform word processing and/or data entry.
• General knowledge of current office practices and procedures and knowledge of the operation of standard office equipment and softwares.
• Ability to prepare and maintain accurate and concise records and reports.
• Ability to work the allocated hours of the position.
LANGUAGE SKILLS
• Ability to communicate effectively with other members of the staff, supervisor, and the public.
• Ability to communicate clearly and concisely in both written and verbal form.
• Must be proficient in workplace English and spelling.

PHYSICAL REQUIREMENTS
• This work requires the occasional exertion of up to 40 pounds of force; work regularly requires sitting, frequently requires standing, speaking or hearing, using hands to finger, handle or feel and repetitive motions and occasionally requires walking, stooping, kneeling, crouching or crawling and reaching with hands and arms.
• Work has standard vision requirements.
• Vocal communication is required for expressing or exchanging ideas by means of the spoken word.
• Hearing is required to perceive information at normal spoken word levels.
• Work is generally in a moderately noisy location (e.g. business office, light traffic).
WORK ENVIRONMENT
• Work is primarily in an office setting.
MINIMUM QUALIFICATIONS:
EDUCATION AND/OR EXPERIENCE REQUIREMENTS
• Bachelor degree in Social Work, human services, or health related field.
• Three years’ experience in a professional position working directly with people with dementia and family/informal caregivers.
• Appointment will be conditional upon successful completion of a caregiver and criminal background check.
Full Pay Range: $59,168 – $$86,556
Expected Pay Range:$59,168 – $70,291
