What we’re looking for:
EHE is seeking a talented and driven Health Navigation Coordinator to manage the coordination of patient recommended screenings/referral appointments while building value in EHE’s brand. The Health Navigation Coordinator will be the liaison between the patients and Health Navigation Center as they assist with the scheduling of post-exam care appointments following our comprehensive health exam, while providing the utmost level of customer service.
As an integral part of the Health Navigation Center, the Health Navigation Coordinator will be tasked with creating unique patient experiences and conversations, identifying gap areas (as it relates to our contracted network and/or member experience), and triaging member and/or facilities related calls that require a more clinical intervention/approach.
This role will also be cross trained in completing Specialty Referrals for patients in need of external consultation as requested by the physician. The Health Navigation Coordinator will work closely with the Director of Health Navigation & post-exam care services to achieve personal and departmental efficiencies and drive EHE’s overall metrics on appointments scheduled.

The salary range for this role is $40,000 – $55,000 and is determined by a number of factors including the candidate’s experience, qualifications and skills.
