Key Responsibilities
Connect with and engage communities as a frontline liaison, facilitating access to health and social services, ensuring culturally competent service delivery.
Increase health knowledge and self-sufficiency among individuals and communities through outreach, education, and resource navigation.
Serve as a bridge between communities and healthcare, government, and social service systems.
Support various initiatives such as chronic disease prevention, COVID-19 education, resource navigation, community outreach, and event coordination.
Staff the Call Center and Resource Coordination Hub, addressing community inquiries and providing necessary services and information.
Essential Duties And Responsibilities
Promote COVID-19-related knowledge and vaccination awareness.
Deliver health information pertinent to chronic disease prevention and health promotion using culturally appropriate terms.
Conduct community outreach, discussing health-related topics, vaccination, and available resources.
Collaborate with various organizations to design and implement health-related activities and interventions.
Connect community members with necessary resources such as food security, healthcare, and employment support.
Systematically record information and maintain communication in a professional manner.
Participate in ongoing professional development and training opportunities.
Requirements
Excellent communication skills with the ability to interact professionally with diverse individuals.
Critical thinking skills and ability to work both independently and collaboratively.
Proficiency in computer skills and willingness to learn new software.
Residency in the City of Chicago preferred.
Must be 18 years or older and authorized to work in the United States.
Desired Qualifications
Prior community engagement or outreach experience.
Familiarity with call center or customer service roles.
Experience working with diverse cultures and socio-economic backgrounds.
Fluency in languages such as Spanish, Polish, Mandarin, Cantonese, or others is advantageous.
Work Environment And Compensation
Remote work, in-office work, or in-person outreach based on project requirements.
Compensation: $22.25 per hour with benefits administered by the employing organization.
