FOLLOWING IS ADDITIONAL INFORMATION:
The Case Manager reports directly to the assigned supervisor with overall program supervision provided by the Clinical Team Manager. Minimum annual service hours are 1200.
QUALIFICATIONS
ESSENTIAL
Education/Licensure
Minimum of a bachelor’s degree in a related behavioral health field with 1,000 hours of supervised post-degree experience with chronically mentally ill persons; Master’s degree preferred.
OR
Minimum of a bachelor’s degree in a field other than behavioral health with 2,000 hours of post-degree experience with chronically mentally ill persons; Master’s degree preferred.
Experience
Typical Physical Demands
Requires driving, sitting, standing, light lifting, bending, and reaching; and the ability to use and operate standard office equipment, including computer, calculator, phone, printer, cell phone, facsimile, etc.
Working Conditions
May require evening or weekend hours depending on program and consumer’s needs.
Proof of COVID-19 vaccination, or submission of ongoing PCR test results required.
