SEP CASEWORKER/CASE MANAGER
LAS ANIMAS COUNTY DEPT. HUMAN SERVICES- SINGLE ENTRY POINT (SEP)

ANNUAL SALARY: Position is a salaried position with an annual wage of $44,741 after successful completion of ninety (90) day probationary period. (Salary will be $3,653.85 per month during 90-day probationary period).

PRIMARY RESPONSIBILITES: A caseworker shall be required to demonstrate competency in the following areas:

• Position provides a variety of social casework services to aged and disabled adults. These services include, but are not limited to, counseling, assessment of client capabilities, case management and Home and Community Based Services (HCBS)/Single Entry

Point and referral to other agencies for additional support

EXAMPLES OF IMPORTANT AND ESSENTIAL DUTIES

• Assess problems and needs of the elderly and disabled. Provides supportive counseling in the areas of health, housing, home management and homemaking, nursing home placement and adjustment, and personal care.

• Position receives referrals for long term care services from a variety of sources including agency Eligibility Technicians, hospital discharge planners, medical and other community resources and relatives.

• Provides personal care, homemaker, and case management services; arranges for services such as adult day care, home health care; coordinates and monitors provision of purchased and provided services to ensure client receives proper care. Determines cost effectiveness of use of alternative services.

• Intervenes in crisis situations to evaluate and stabilize situations.

• Establishes community relations and consults with other agencies and professionals, homemakers, home health agencies, nursing home staff, mental health staff to coordinate services and meet special needs of clients.

• Prepares and maintains client records, completes required forms, participates in client staffing conferences and workshops.

• Applicant must have knowledge of social casework techniques, as well as knowledge of the problems of the elderly, physically disabled, and other at-risk adults; ability to analyze and diagnose problems of aged and disabled persons and to develop programs to enable aged and disabled adults to function and become self-sufficient. Applicant must also have the ability to establish sound rapport with clients and professionals, and to be sensitive to individual values and needs.

• Applicant will be responsible for other duties as assigned based on needs of SEP Unit and agency.

REQUIREMENTS:

• Caseworker must possess a valid driver’s license, reliable transportation, and specified car insurance.
• Caseworker must be willing to travel to rural areas of county and out of town as needed.
• Caseworker must be able to successfully pass a criminal background check and drug screen.
• Caseworker must be able to work after hours or on call if determined by supervisory staff.
• Caseworker must be able to attend required out of town trainings and staff development activities.
• Applicants will be required to provide a driving history if position is offered.
• A professional and appropriate dress code is in effect for all employees of the office.

EDUCATION: This position requires a Bachelor’s Degree in one of the human behavioral science fields such as human services, nursing, social work, psychology, sociology etc.

PREFERRED QUALITIES:

• Bilingual in English and Spanish helpful but not required.
• Knowledge of geriatrics.
• Medical knowledge and medical terminology helpful but not required.

PHYSICAL DEMANDS: Position requires substantial movement with respect to repetitious standing and sitting within the office area.
On occasion, may be required to lift light to moderately heavy loads.
