UH: Emergency Room Core (Full-time. Day Shift)-

The Clinical Referral Nurse participates in the evaluation of patient outcomes related to clinical test results and clinical referrals. Case manages complex cases or designated population of patients seeking emergency services. Advises, counsels and educates within the parameters of the Registered Nurse (RN) scope of practice and documents appropriately. Evaluates patient for clinical appropriateness of care and services. Makes appropriate healthcare referrals as indicated. Provides clinical education and clarification of discharge and aftercare instructions. Leads multidisciplinary groups to facilitate collaboration with other disciplines in the attainment of outcomes across he continuum of care. Directs communication with the physician or clinical support staff to elicit clinical information and facilitate aftercare planning. Promotes patient satisfaction through access, quality care, education and service. Assists patient in obtaining appropriate clinical care of abnormal or delayed ancillary clinical tests. Reaches appropriate clinical decisions in critical and complex situations. Acts as clinical Liaison between patients and physicians. Coordinates aftercare in conjunction with physicians, nurse and other clinical staff, payers, patients and their families. Performs other duties as needed.
Bachelor’s Degree in Nursing required. Minimum three years of Emergency Room and Case Management experience required. Minimum two years of Utilization Review or discharge planning experience preferred. Active California Registered Nurse (RN) License required. Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certification from the American Heart Association required.
Working knowledge or regulatory requirements and accreditation standards required. Able to use computer, printer, and software programs relevant to the position; troubleshoot and calibrate patient care equipment, perform technical patient care activities. Able to related and communicate positively, effectively and professional with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; manage, lead. Supervise, teach, collaborate and accept direction. Able to communicate effectively in English in person, in writing and on the telephone with professional quality; think critically; work independently with minimal supervision; perform basic math functions; manage multiple assignments effectively; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail. Distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the workplace and patient care; see adequately to read computer screens, medical records, and written documents necessary to the position; discern temperatures variances through touch.
