Job Description

• Position Summary:

Reports to the Region 2 Lead Community Health Worker. The Region 2 community health worker will link priority neighborhood members to health and social services, engage in intra- and inter-agency collaborative efforts to facilitate access to services and improve the quality and cultural competence of service delivery. The community health worker will build individual and community capacity by increasing health knowledge and self-sufficiency.

• Minimum Qualifications:
• High school diploma or equivalent required.
• One year of experience working or volunteering with community based programs in Region 2.

• Position Responsibilities:

30%Community Advocacy

• Advocate for local health and social needs for the priority neighborhood residents and community members.
• Help clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible.
• Assist clients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services.
• Facilitate communication and coordinate services between providers.
• Motivate patients to be active, engaged participants in their health.
• Facilitate communication and client empowerment in interactions with health care/social service systems.
• Effectively work with people (staff, clients, doctors, agencies, etc) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions.
• Build and maintain positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff.
• Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place.

30%Intra- and inter-agency collaboration (Testing & Case Management)

• Provide rapid testing in the field and linkage to care after proper training and certification.
• Work as part of a team in Region 9 to coordinate patient care across health systems and to navigate community resources.
• Collaborate with clinical team to inform quality improvement efforts.
• Conduct casework interviews to identify patient’s psychosocial, economic and physical needs.
• Address needs by referring into and/or directly contacting existing community programs and social support organizations, such as shelters, substance abuse programs, job assistance programs, etc.
• Assist patients in completing governmental applications such as Medicaid, Medicare, LaChip, etc.
• Maintains list of up-to date community resources for clinic and community use.
• Conduct home visits for clients who need additional assistance.
• Provide informal counseling support and follow up.
• Accurately and appropriately document communications, observations, interventions and referrals in the electronic medical record.

30%Community health education and outreach

• Disseminate information relevant to the priority neighborhoods in Region 9 at community events and conferences.
• Assist with outreach efforts in Region 9 to inform individuals, families, and local agencies about health and wellness services offered by the collaborative partner agencies.
• Distribute culturally appropriate health information resources in Region 9 and refer patients to additional education resources.
• Assist with coordinating group health education sessions and outreach events on topics of interest to the community and/or clinic population.
• Assist with staffing health fair events.
• Develop, track and maintain database of community outreach activities for grant reporting purposes.
• Participate in local community health worker network meetings and community health improvement activities.

9%Medical Visit Support

• Responsible for communication support between patient and provider.
• Assist patient with filling out documents or forms.
• Responsible for ensuring patient understand follow-up care, medication needs, and health education forms as needed.
• Responsible for keeping informed with documents pertaining to appropriate procedures and bulletins from the Office of Minority Health.

1%Other

• Continuously expand knowledge and understanding of community resources, services and programs provided; human relations and the procedures used in dealing with the public as part of a service or program; volunteer resources and the practices associated with using volunteers, operations, functions, policies and procedures associated with the department or program area, procedures and resources available to handle new, unusual or different situations.

• Approximate Work Time Distribution

Position is considered full-time with a minimum 40 hours of work per week.

• General Work Environment

The incumbent should be able to operate a personal computer and have experience working with data systems and electronic medical records systems. Must have the ability to function in a high pressure environment and to meet stringent deadlines in completing tasks and projects. Superior verbal and written communication skills are required. The incumbent should be able to observe and assess programs and services and be able to formulate and communicate action plans, schedules, timelines related to project activities.Approximately 60% of work shall be completed in an office setting in Region 9. The office is located at 1206 J.W Davis Drive Ste.105 Hammond, LA 70401. Periods of standing, sitting, using computers, scanning documents, talking on the phone, interacting with related agency representatives, and interacting with potential clients is expected. Ability to lift and carry a minimum of 10 pounds and walk at least 2-3 miles periodically is also expected.Approximately 40% of work shall be completed in community settings, training Lead and Community Health Workers in community settings, using a boots on the ground approach.Ability to move about the facility, with or without assistance, is required.Regional and occasional statewide travel required. A valid driver’s license and access to reliable transportation for regional and statewide travel is required. Up to 20% of work time may be spent travelling off campus or spent working before or after posted office hours and on weekends, including on-call assignments.Job Type: Full-timePay: From $40,000.00 per yearBenefits:
