Reports to the Region 2 Lead Community Health Worker. The Region 2 community health worker will link priority neighborhood members to health and social services, engage in intra- and inter-agency collaborative efforts to facilitate access to services and improve the quality and cultural competence of service delivery. The community health worker will build individual and community capacity by increasing health knowledge and self-sufficiency.

Position is considered full-time with a minimum 40 hours of work per week.
The incumbent should be able to operate a personal computer and have experience working with data systems and electronic medical records systems. Must have the ability to function in a high pressure environment and to meet stringent deadlines in completing tasks and projects. Superior verbal and written communication skills are required. The incumbent should be able to observe and assess programs and services and be able to formulate and communicate action plans, schedules, timelines related to project activities.Approximately 60% of work shall be completed in an office setting in Region 9. The office is located at 1206 J.W Davis Drive Ste.105 Hammond, LA 70401. Periods of standing, sitting, using computers, scanning documents, talking on the phone, interacting with related agency representatives, and interacting with potential clients is expected. Ability to lift and carry a minimum of 10 pounds and walk at least 2-3 miles periodically is also expected.Approximately 40% of work shall be completed in community settings, training Lead and Community Health Workers in community settings, using a boots on the ground approach.Ability to move about the facility, with or without assistance, is required.Regional and occasional statewide travel required. A valid driver’s license and access to reliable transportation for regional and statewide travel is required. Up to 20% of work time may be spent travelling off campus or spent working before or after posted office hours and on weekends, including on-call assignments.Job Type: Full-timePay: From $40,000.00 per yearBenefits:
