JOB SUMMARY

The Home Health Nurse works under the general direction of the Home Health Director as a member of the health care team to provide nursing care for homebound patients. The nurse utilizes the process of assessment, planning, implementation and evaluation to assure the delivery of health care activities.
Variable Schedule; Day shift, includes weekends, and a rotating weeknight and weekend on call.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: Graduate of an accredited school of nursing.
Preferred/Optional: Graduate of an accredited school of nursing in community health.
EXPERIENCE
Minimum Required: None
Preferred/Optional: Minimum of two years acute care nursing experience. One year of home health nursing experience in a certified home health.
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: Current State of Wisconsin Registered Nurse License or Nurse Compact License. Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire.
Preferred/Optional: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
